目的分析超声检查在右下腹部急性疼痛临床诊断中的应用价值。方法回顾性分析汕头市澄海区人民医院2014年1月-2015年12月间收治的87例右下腹部急性疼痛患者病历资料,全部患者均于就诊时行彩色多谱勒超声检查,以病理或后续治疗结果为金标准,对比超声检查诊断结果与金标准诊断结果间的差异性,据此计算超声检查在右下腹部急性疼痛诊断中的特异性、敏感性、准确率、误诊率与漏诊率。结果超声检查明确诊断80例,诊断总准确率为91.95%,总漏误诊率为8.05%;超声检查诊断结果与金标准诊断结果比较,各疾病差异均无统计学意义(P>0.05)。超声诊断卵巢囊肿蒂扭转与胃十二指肠穿孔的特异性、敏感性及准确率最高均达100.00%。结论超声检查在右下腹部急性疼痛临床诊断中具有准确率高、操作便捷、重复性好的优势,可为右下腹部急性疼痛的临床鉴别与诊断提供可靠依据。
Objective To analyze the clinical value of ultrasonography in the diagnosis of acute right lower quadrant abdominal pain. Methods The clinical data of 87 patients with acute right lower quadrant abdominal pain who were admitted to Chenghai District People’s Hospital of Shantou from January 2014 to December 2015 were retrospectively analyzed. All patients underwent color Doppler ultrasonography at the time of diagnosis, Follow-up treatment results for the gold standard, the contrast between the diagnostic results of ultrasonography and gold standard diagnostic results of the difference between the calculated ultrasound diagnosis of acute right lower quadrant pain in the specificity, sensitivity, accuracy, misdiagnosis rate and missed diagnosis rate . Results Eighty cases were diagnosed by ultrasonography. The total diagnostic accuracy rate was 91.95% and the total misdiagnosis rate was 8.05%. There was no significant difference between the diagnostic results of ultrasonography and gold standard diagnostic results (P> 0.05). Ultrasound diagnosis of ovarian cyst torsion and gastroduodenal perforation specificity, the highest sensitivity and accuracy of up to 100.00%. Conclusion Ultrasonography has the advantages of high accuracy, convenient operation and good repeatability in the clinical diagnosis of acute right lower quadrant abdominal pain, which can provide a reliable basis for the clinical identification and diagnosis of acute right lower quadrant abdominal pain.
