目的 探讨细胞周期调控因子蛋白表达在大肠癌发生发展中的作用及其对预后影响。方法 采用免疫组化染色对 113例大肠癌、14例大肠腺瘤、11例腺瘤癌变组织中CyclinD1、P16和PCNA进行了检测。结果 大肠癌、腺瘤和癌变CyclinD1、P16以及PCNA阳性表达率与正常大肠粘膜相比均相差显著 (P <0 .0 5 ) ,腺瘤癌变CyclinD1表达明显增高 ,大肠癌P16表达明显降低。大肠癌、腺瘤癌变的PCNA较腺瘤组显著增高 (P <0 .0 1)。CyclinD1、PCNA过度表达者 ,其 5年生存率均显著降低。结论 CyclinD1过度表达与P16基因突变失活可能加速细胞周期转化 ,促进大肠癌的发生 ;CyclinD1过度表达在大肠癌早期形成过程中可能起一定作用。CyclinD1、PCNA过度表达提示为大肠癌预后不良的危险因素。
Objective To investigate the role of cell cycle regulator protein expression in the development of colorectal cancer and its effect on prognosis. Methods Immunohistochemical staining was used to detect CyclinD1, P16 and PCNA in 113 cases of colorectal cancer, 14 cases of colorectal adenoma and 11 cases of adenoma. Results The positive expression rates of CyclinD1, P16 and PCNA in colorectal cancer, adenoma and carcinoma were significantly different from those in normal colorectal mucosa (P < 0.05). The expression of CyclinD1 in adenoma was significantly increased and the expression of P16 in colorectal cancer was significantly decreased. The PCNA of colorectal cancer and adenoma was significantly higher than that of adenoma group (P < 0.01). The 5-year survival rate of CyclinD1 and PCNA overexpression was significantly reduced. Conclusion The overexpression of CyclinD1 and inactivation of P16 gene may accelerate cell cycle transformation and promote the occurrence of colorectal cancer. Overexpression of CyclinD1 may play a role in the early formation of colorectal cancer. Overexpression of Cyclin D1 and PCNA suggest a risk factor for poor prognosis of colorectal cancer.
