目的探讨逸搏心律伴室房双径路传导心电图特征和发生机制。方法选取11例逸搏心律伴室房双径路传导患者的心电图资料进行回顾性分析。结果 11例逸搏心律包括交界性7例、室性4例,逸搏频率相对匀齐,其后跟随P波,P波呈两种形态的7例,单一形态的4例。均存在长、短两种R-P间期,差值明显大于0.06s,长、短RP间期最大增量变化在0.11~0.38s(平均0.23±0.09s),3例伴随单个心室回波,1例出现房室结折返性心动过速。结论室房双径路传导是一种少见的心电现象,通过体表心电图合理分析,可以对逆向型双径路做出初步诊断,为下一步实施电生理检查和治疗提供指导依据。
Objective To investigate the characteristics and mechanism of Yaping rhythm with dual-path pathway ECG. Methods Eleven patients with esophageal atrial arrhythmia and ventricular conduction pathways were retrospectively analyzed. Results Eleven patients underwent esophageal atrial rhythm, including 7 cases of borderline and 4 cases of ventricular aneurysm. The frequency of escaping was relatively homogeneous. Followed by P wave, P wave showed two kinds of morphology in 7 cases and single morphology in 4 cases. There were two kinds of long and short RP interval, the difference was significantly greater than 0.06s, long and short RP interval maximum incremental changes in 0.11 ~ 0.38s (mean 0.23 ± 0.09s), 3 patients with a single ventricular echo, 1 Cases of atrioventricular nodal reentrant tachycardia. Conclusions The dual-path pathway is a rare electrocardiogram in our hospital. Through the rational analysis of body surface electrocardiogram, we can make a preliminary diagnosis on the double-path reverse biofeedback and provide the basis for further electrophysiological examination and treatment.
