由肾皮质部的间质纤维化所引起的肾小球滤过率受损,可见于各种炎症性和非炎症性肾脏病变。对近端肾小管的形态学测量研究,能取得有关间质纤维化影响肾小球滤过率的途径的更多知识。40例肾活组织其中30例为各种肾脏病变(不包括急性肾功能衰竭),平均37.1±15.8岁,10例无肾脏病理学改变者作对照,年龄为16~46岁。分别测量并计算近端肾小管总面积,肾小管上皮细胞面积和肾皮质部的间质体积,同时测定血肌酐浓度和内生肌酐清除率,然后分析上述各参数之间的关系,经过统计学处理后所得结果显示:①血肌酐浓度与近端肾小管总面积及肾小管上皮细胞面积之间呈显著的负相关。②肾皮质部的间质体积与近端肾小管总面积和近端肾小管上皮细胞面积之间呈显著的负相关。③血肌酐浓度与肾皮质部的间质宽度呈明显的正相关。④患者年龄与肾皮质部的间质体积之间呈显著的正相关,且与血肌酐浓度也明显有关。⑤未发现患者年龄与近端肾小管总面积以及近端肾小管上皮细胞面积之间有显著关系。
Glomerular filtration rate caused by interstitial fibrosis of the renal cortex is impaired and is found in a variety of inflammatory and non-inflammatory renal pathologies. Morphological measurements of proximal tubules can gain more knowledge about the pathways by which interstitial fibrosis affects glomerular filtration rate. Thirty cases of 40 cases of renal biopsy were all kinds of nephropathy (excluding acute renal failure), with an average of 37.1 ± 15.8 years old, 10 cases without renal pathological changes as controls, aged from 16 to 46 years old. The total proximal tubule area, the tubular epithelial cell area and the interstitial volume of the renal cortical area were measured and calculated, and the serum creatinine concentration and endogenous creatinine clearance rate were measured. The relationships among the above parameters were analyzed and statistically analyzed The results obtained after treatment showed: ① serum creatinine concentration and proximal tubule area and tubular epithelial cell area was significantly negatively correlated. ② The interstitial volume of renal cortex was significantly negatively correlated with the total proximal tubule area and proximal tubular epithelial cell area. ③ serum creatinine and renal interstitial cortical width was significantly positively correlated. ④ There was a significant positive correlation between age of patients and interstitial volume of renal cortex, and the serum creatinine concentration was also significantly related. ⑤ No significant correlation was found between patient age and proximal tubule total area and proximal tubule epithelial cell area.
