目的:在2型糖尿病患者中评估10g单尼龙丝、128Hz分级音叉、振动觉阈值(vibration perception thresholds,VPT)测定在糖尿病周围神经病变筛查诊断中的应用价值。方法:选择符合标准的2型糖尿病患者180例,其中有症状组116例,无症状组64例。所有对象用上述检查方法进行感觉功能评价,同时进行神经电生理检查,比较各种检查方法的阳性检出率,以神经传导速度(NCV)作为标准,比较3种简易方法与NCV检查的相关性,并计算各种检查方法的敏感性和特异性。结果:在有症状和无症状糖尿病患者中,各种检查方法都出现了一定的阳性检出率,尤其是VPT在无症状患者中的阳性率较高(47.7%);3种简易方法都与NCV具有较显著的相关性;且与NCV阳性结果相比具有较好的敏感性和特异性。结论:感觉功能评价在早期发现糖尿病病人周围神经损害方面具有很好的价值和临床应用前景。
OBJECTIVE: To evaluate the value of 10 g single nylon filament, 128 Hz graded tuning fork, and vibration perception thresholds (VPT) in the screening of diabetic peripheral neuropathy in type 2 diabetic patients. Methods: A total of 180 patients with type 2 diabetes mellitus were selected, including 116 symptomatic patients and 64 asymptomatic patients. All subjects evaluated the sensory function by the above examination method, and performed neuroelectrophysiological examination at the same time, and compared the positive detection rate of various examination methods with the nerve conduction velocity (NCV) as the standard to compare the correlation between the three simple methods and NCV examination , And calculate the sensitivity and specificity of various test methods. Results: In both symptomatic and asymptomatic diabetic patients, a variety of examination methods showed positive detection rate, especially the positive rate of VPT in asymptomatic patients (47.7%). Three simple methods were NCV has a more significant correlation; and NCV positive results compared with good sensitivity and specificity. Conclusion: The evaluation of sensory function has good value and clinical application in the early detection of peripheral nerve damage in diabetic patients.
