目的 观察异丙酚联合米索前列醇阴道给药在无痛人工流产术中的作用。方法 将200例早孕未产妇女和/或有剖宫产史妊娠妇女随机分为试验组和对照组,每组100例。对照组静脉推注异丙酚,按2 5mg/kg给药诱导麻醉,孕妇意识消失后即开始手术,试验组术前2h阴道给米索前列醇200μg(1片),其余操作同对照组。术中静脉开放,根据情况维持麻醉,同时观察麻醉效果、宫颈扩张、子宫收缩、出血量及人工流产综合征发生情况。结果 两组对象麻醉效果有效率均为100%,无一例发生人工流产综合征。宫颈扩张情况试验组顺利通过6号黑格尔扩张器者有96例(96%),而对照组只有8例(8%)。子宫收缩幅度,试验组术后子宫收缩1~2cm有79例(79%),对照组只有30例(30%)。阴道出血情况试验组出血小于15ml有75例(75%),对照组有25例(25%)。在上述3方面两组相比较差异有统计学意义(P<0 .01)。结论 异丙酚联合米索前列醇阴道给药,镇痛效果良好,宫颈易扩张,阴道出血量少,能降低人工流产并发症发生。
Objective To observe the effect of propofol combined with misoprostol in painless artificial abortion. Methods Two hundred pregnant women without early pregnancy and / or pregnant women with history of cesarean section were randomly divided into experimental group and control group, with 100 cases in each group. In the control group, propofol was injected intravenously at a dose of 25 mg / kg to induce anesthesia. After the pregnant women disappeared, the operation was started. Conclusion Propofol combined with misoprostol vaginal administration, analgesic effect is good, cervical dilatation, vaginal bleeding less, can reduce the incidence of induced abortion.
