目的探讨重型颅脑损伤患者的动态脑电图(AEEG)表现与临床的关系和意义。方法对过去2年间64例重型颅脑损伤患者进行了AEEG监测和回顾性分析。结果 64例患者的AEEG均为异常,轻度异常17例均存活(100%),中度异常26例中死亡2例(7.7%),重度异常21例中死亡12例(57.1%)。AEEG改变以广泛性异常为主,多呈弥漫性或暴发性δ波,颅脑损伤程度越重,AEEG异常越明显,表现为频率越慢,波幅越低甚至呈平坦波改变,预后也越差。根据AEEG监测检出病样波的情况帮助临床调整用药。结论分析重症颅脑损伤患者的AEEG表现对判断病情和预后、诊断脑死亡以及指导临床用药有较大的意义。
Objective To investigate the relationship and significance of dynamic electroencephalogram (AEEG) manifestations in patients with severe craniocerebral injury. Methods Sixty-four patients with severe craniocerebral injury over the past two years underwent AEEG surveillance and retrospective analysis. Results All 64 patients had abnormal AEEG, 17 of them survived (100%) with mild abnormality, 2 of 26 (7.7%) with moderate abnormality, and 12 (57.1%) of 21 with severe abnormality. The changes of AEEG are mainly diffuse or outbreak, mostly diffuse or outbreak of δ waves. The more serious the brain injury, the more obvious AEEG abnormalities, the slower the frequency, the lower or even the flat wave, the worse the prognosis . According to AEEG detection detected disease-like wave to help adjust the clinical medication. Conclusion The analysis of AEEG findings in patients with severe craniocerebral injury is of great significance in judging the condition and prognosis, in diagnosing brain death and in guiding clinical medication.
