异丙酚联合氧化亚氮吸入麻醉用于人工流产术的临床观察

目的 评价异丙酚联合氧化亚氮 (笑气 )吸入麻醉用于人工流产术的疗效和安全性。方法  2 4 0例自愿要求终止妊娠的健康孕妇 ,随机分为四组 ,各 6 0例。P F组 :芬太尼 1μg/kg与异丙酚 2mg/kg静注 ,术中根据效果追加异丙酚 ;N2 O组 :手术前 2min吸入 5 0 %N2 O O2 混合气 ,手术结束前 1min停吸 ;P N组 :同N2 O组法吸入混合气 ,同时静注 1 0mg/kg异丙酚 ,术中酌情追加 ;对照组 (B组 )未用麻醉。 结果 P N组呼吸循环稳定 ,与P F组差异有显著意义 (P <0 0 5 )。P F组、P N组无痛率 10 0 % ,明显高于对照组 (2 6 7% )和N2 O组 (78 3% ) (P <0 0 1)。初醒时间 :N2 O组 2 71± 1 12min和P N组 3 11± 1 17min短于P F组5 6 4± 1 35min(P <0 0 5 )。P N组无人流综合征 (PASS)发生 ,明显低于对照组 (13例 ) ,P N组无呼吸暂停 ,明显低于P F组 (9例 ) (P <0 0 1)。结论 异丙酚联合笑气吸入麻醉行人工流产术安全高效 ,呼吸循环稳定、清醒快、并发症少。 Objective To evaluate the efficacy and safety of propofol combined with nitrous oxide inhalation anesthesia for artificial abortion. Methods Twenty-four healthy pregnant women who voluntarily requested termination of pregnancy were randomly divided into four groups of 60 cases. PF group: fentanyl 1μg / kg and propofol 2mg / kg intravenously, according to the effect of intraoperative additional propofol; N2O group: 2Omin before surgery inhalation of 50% N2 O O2 mixture, 1min before the end of surgery Inhalation; PN group: with N2 O group inhalation of gas mixture, while intravenous injection of propofol 10mg / kg, intraoperative discretion; control group (B) without anesthesia. Results The respiratory cycle was stable in P N group, which was significantly different from that in P F group (P <0.05). The painless rates in the P F and P N groups were significantly higher than those in the control group (26.7%) and N2O group (78.3%) (P <0.01). Wake-up time was shorter in N2O group than in 2171 ± 12min group and Pn group (11117min) than that in PF group (P <0 05). The incidence of no-flow syndrome in P N group was significantly lower than that in control group (13 cases). P N group had no apnea and was significantly lower than that in P F group (9 cases) (P <0.01). Conclusions Propofol combined with nitrous oxide inhalation anesthesia is safe and effective in artificial abortion. It has stable respiratory cycle, rapid awakeness and less complications.
