目的探讨肝硬化合并肝癌及单纯肝癌患者不同Child-Pugh分级时补体C3的表达意义。方法选取肝癌患者121例,无肝病证据的健康人37例为对照组,均测定补体C3含量。将肝癌患者分为肝硬化合并肝癌组及单纯肝癌组,并根据Child-Pugh评分分级分组,并比较两组间同等Child-Pugh分级状态时补体C3水平的差异。将单纯肝癌组与对照组进行补体C3水平比较。结果肝癌两组中血清补体C3水平随着Child-Pugh分级由A级至C级变化而逐渐降低,组间比较,A级、B级时单纯肝癌组补体C3水平高于肝硬化合并肝癌组,差异均有统计学意义(P<0.05),C级时两组比较,差异无统计学意义(P>0.05)。单纯性肝癌组与对照组比较,A级患者血清补体C3水平高,但差异无统计学意义(P>0.05),B级及C级患者,补体水平逐渐降低,与对照组相比差异有统计学意义(P<0.05)。结论血清补体C3与肝功能损伤严重程度相关,结合Child-Pugh分级可更好地反映肝癌及肝硬化合并肝癌时肝脏储备功能级体液免疫水平,对临床诊治有重要意义。 Objective To investigate the expression of complement C3 in patients with cirrhosis complicated with hepatocellular carcinoma and simple liver cancer under different Child-Pugh classification. Methods Totally 121 patients with liver cancer and 37 healthy people without evidence of liver disease were selected as the control group. The levels of complement C3 were determined. Patients with liver cancer were divided into liver cirrhosis with hepatocellular carcinoma and simple hepatocellular carcinoma, and were grouped according to Child-Pugh score. The difference of complement C3 level between the two groups was compared in the same Child-Pugh classification. The levels of complement C3 in HCC group and control group were compared. Results The levels of serum complement C3 in patients with HCC were gradually decreased as the level of Child-Pugh changed from A to C. The levels of complement C3 in patients with HCC were higher than those in patients with hepatocirrhosis, (P <0.05). There was no significant difference between the two groups at C level (P> 0.05). Compared with the control group, the levels of serum complement C3 in patients with simple liver cancer were significantly higher than those in patients with normal controls (P> 0.05). The levels of complement in patients with grade B and C decreased gradually Significance (P <0.05). Conclusions Serum complement C3 is associated with the severity of liver injury. Combined with Child-Pugh classification, it can better reflect the level of liver reserve function-level humoral immunity in patients with liver cancer and liver cirrhosis complicated with liver cancer, which is of great significance for clinical diagnosis and treatment.
