目的评价老年食管癌贲门癌患者的临床病理特点、围手术期治疗和手术治疗效果。方法对安阳市肿瘤医院2010年6月至2011年9月收治的90例70岁以上老年食管癌、贲门癌患者的临床资料进行回顾性分析。结果 70岁以上食管癌贲门癌患者总切除率为94.4%(85/90)。术后并发症发生吻合口瘘3例,占3.3%,消化功能紊乱6例,占6.7%,肺不张、胸腔包裹性积液、肺部感染4例,占4.4%,哮喘发作3例,占3.3%,全组无死亡病例;随访1年,由于癌转移死亡4例,86例存活;随访2年死于癌转移6例。结论对食管癌、贲门癌老年患者进行充分的术前准备及选择恰当合理的手术途径,同时加强患者围手术期的管理,是提高手术成功率,降低术后并发症,提高患者生存质量的关键。
Objective To evaluate the clinicopathological characteristics, perioperative treatment and surgical treatment of elderly patients with esophageal and cardiac cancer. Methods The clinical data of 90 elderly patients with esophageal and cardiac cancer who were admitted to Anyang Cancer Hospital from June 2010 to September 2011 were analyzed retrospectively. Results The total resection rate was 94.4% (85/90) in patients with esophageal and cardiac cancer over 70 years old. Postoperative complications occurred in 3 cases of anastomotic fistula, accounting for 3.3%, digestive dysfunction in 6 cases, accounting for 6.7%, atelectasis, pleural fluid package, 4 cases of pulmonary infection, accounting for 4.4%, 3 cases of asthma attacks, Accounting for 3.3%. There were no deaths in the whole group. One year after follow-up, 4 patients died of cancer metastasis and 86 patients survived. Two patients died of cancer metastasis in 2 years. Conclusion The adequate preoperative preparation for esophageal and cardia cancer elderly patients and the selection of appropriate and reasonable surgical approaches, as well as strengthening the perioperative management of patients, is the key to improve the success rate of surgery, reduce postoperative complications and improve the quality of life of patients .
