目的 研究粘附分子 Ecadherin 在胆囊腺癌中的表达及与胆囊腺癌分化、侵袭、转移和预后的关系。方法 应用免疫组织化学方法对57 例原发性胆囊腺癌、37 例慢性胆囊炎石蜡包埋标本进行 Ecadherin 半定量检测。结果 分别有 45 例(78.9% )胆囊腺癌组织及 11 例(29.7% )慢性胆囊炎组织中 Ecadherin 呈阴性或不同程度表达减弱。 T1、 T2 期癌 Ecadherin 表达阳性率明显高于 T3、 T4 期癌( P< 0.05)。 Ecadherin 表达阳性者其2 年生存率(27.3% )明显高于表达阴性者(8.3% ),差异有显著性( P< 0.05)。结论 Ecadherin 表达与胆囊腺癌 P T N M 分期呈负相关,其表达水平可作为胆囊癌患者预后判断的生物学指标。
Objective To investigate the expression of adhesion molecule Ecadherin in gallbladder adenocarcinoma and its relationship with differentiation, invasion, metastasis and prognosis of gallbladder adenocarcinoma. Methods Immunohistochemistry was used to semi-quantitatively detect E-cadherin in paraffin-embedded specimens of 57 cases of primary gallbladder adenocarcinoma and 37 cases of chronic cholecystitis. Results 45 cases (78.9%) of gallbladder adenocarcinoma tissues and 11 cases (29.7%) of chronic cholecystitis were negative for E-cadherin or different degrees of expression. The positive rates of E-cadherin expression in T1 and T2 tumors were significantly higher than those in T3 and T4 tumors (P<0.05). The 2-year survival rate (27.3%) of patients positive for E-cadherin expression was significantly higher than that of negative expression (8.3%), with a significant difference (P<0.05). Conclusion The expression of Ecadherin is negatively correlated with P T N M staging of gallbladder adenocarcinoma. The expression level of Ecadherin can be used as a biological indicator of prognosis of gallbladder carcinoma.
