目的：观察美施康定、路泰对重度癌痛的止痛效果及对患者生活质量的影响。方法：145例经病理证实的中晚期重度癌痛病人，83例服用美施康定（A组），62例服用路泰（B组），初始剂量均为30mg口服．每12h一次；据实际疗效来调整用量．每次递增30mg．直到达到满意的止痛效果。结果：美施康定、路泰其镇痛有效率分别为：95．2％，93．5％。完全缓解率分别为81．9％，80．6％。镇痛有效时间分别为：9.1±4．1h，8．7±4．4h.西药镇痛效果及镇痛有效时间经统计学处理无显著位差异（P＜0．05）。治疗后分别有62人（74．7％），47人（7．8％）生活质量较前提高一个等级，治疗前后患者生活质量存在着显著性差异（P＜0．01）。两药对骨疼、内脏疼、软组织浸润痛效果好．对神经痛效果欠佳。神经痛与其它类型疼痛间其镇痛效果存在着显著性差异（P＜0．01）。西药的主要不良反应均为便秘．恶心、呕吐、嗜睡、排尿困难等，治疗剂量未发现精神依赖发生。结论：美施康定或路泰对重度癌痛止痛效果好，镇痛时间长．副作用小；改善了患者的生活质量，应作为治疗重度癌痛的首选药物。
OBJECTIVE: To observe the analgesic effect of metstaining and lutai on severe cancer pain and its effect on the patient’s quality of life. METHODS: One hundred and forty-five patients with pathologically confirmed severe cancer pain in the middle and late stages were enrolled in 83 patients with Meskodine (group A) and 62 patients with Lutai (group B). The initial dose was 30 mg orally. Once every 12h; According to the actual effect to adjust the dosage. Each time increment 30mg. Until it reaches a satisfactory analgesic effect. Results: The effective rates of pain relief by Methinking and Lutaiqi were 95.2% and 93.5% respectively. The complete remission rate was 81.9% and 80.6%, respectively. The effective analgesic time was: 9.1±4.1h, 8.7±4.4h. Western medicine analgesic effect and effective time of analgesia were not statistically significant (P<0.05). After treatment, there were 62 (74.7%) and 47 (7.8%) patients whose quality of life was improved by one grade. The quality of life of patients before and after treatment was significantly different (P<0.01). The two drugs have good effects on bone pain, visceral pain and soft tissue infiltration. The effect on neuralgia is not good. There was a significant difference in the analgesic effect between neuralgia and other types of pain (P<0.01). The major adverse reactions of western medicine are constipation. Nausea, vomiting, drowsiness, difficulty urinating, etc. No dose of mental dependence was found at therapeutic doses. 4. Conclusion: Methinkine or Lutai is effective for severe cancer pain relief and long time for pain relief. Small side effects; improve the quality of life of patients, should be used as the drug of choice for the treatment of severe cancer pain.
