目的本研究旨在探讨有全数淋巴结检查的胃癌病例中STAT1与Survivin的相关性以及两者与淋巴结转移、浸润深度的关系及其预后意义。方法收集江汉大学附属医院1988-2003年胃癌根治性手术切除,有完整病理学检查资料的标本140例。经溶脂法检查全数切除淋巴结、4μm间断连续切片、常规HE染色和免疫组化染色,确定有无淋巴结转移。构建胃癌组织微阵列,免疫组织化学染色检测STAT1蛋白和Survivin蛋白在胃癌组织中的表达情况。数据经SPSS12.0统计学软件进行Spearman等级相关分析,Kaplan-Meyer法Log-rank检验及Cox模型回归分析(SPSS12.0)。结果构建的140例胃癌组织微阵列中,符合分析要求的共110例(组织微阵列利用率为78.6%)。110例中共检出淋巴结7079枚,平均64.4枚/例。其中89例有淋巴结转移,共检出转移淋巴结1679枚。Survivin阳性率为52.7%(58/110),STAT1阳性率为40%(44/110)。STAT1表达与Survivin表达负相关(r=-0.19,P=0.04)。STAT1与浸润深度(r=-0.21,P=0.02)负相关。Sur- vivin表达与UICC N分期(r=-0.24,P=0.01)及组织学分型(r=-0.21,P=0.03)负相关。两者和生存无关。Cox模型回归多因素分析,UICC淋巴结分期与预后相关(X2=4.85,P=0.028)。结论胃癌STAT1蛋白表达与Survivin蛋白表达负相关。两者与生存无关。STAT1是推测进展期胃癌的分子标志物。Survivin是推测胃癌淋巴结转移的分子标志物。UICC胃癌淋巴结分期是重要的预后指标。
Objective This study aimed to investigate the correlation between STAT1 and Survivin in gastric cancer with all lymph node examinations and the relationship between STAT1 and lymph node metastasis and depth of invasion and its prognostic significance. Methods A total of 140 specimens of patients with gastric cancer undergoing radical resection from 1988 to 2003 and with complete pathological examination were collected from the Affiliated Hospital of Jianghan University. The lymphadenectomy was performed to examine the total lymph node dissection, serial sections of 4μm were cut, routine HE staining and immunohistochemical staining were performed to determine whether there was lymph node metastasis. The gastric cancer tissue microarray was constructed and the expression of STAT1 protein and Survivin protein in gastric cancer tissues were detected by immunohistochemical staining. Spearman rank correlation analysis, Kaplan-Meyer Log-rank test and Cox regression analysis were used to analyze the data by SPSS12.0 statistical software (SPSS12.0). RESULTS: Of the 140 gastric cancer tissue microarrays constructed, 110 were eligible for analysis (tissue microarray utilization was 78.6%). 110 cases of the Chinese Communist Party found 7079 lymph nodes, an average of 64.4 pieces / case. Among them 89 cases had lymph node metastasis, and 1679 metastatic lymph nodes were detected. Survivin positive rate was 52.7% (58/110), STAT1 positive rate was 40% (44/110). STAT1 expression was negatively correlated with Survivin expression (r = -0.19, P = 0.04). STAT1 was negatively correlated with the depth of invasion (r = -0.21, P = 0.02). Survivin expression was negatively correlated with UICC N stage (r = -0.24, P = 0.01) and histological type (r = -0.21, P = 0.03). Both have nothing to do with survival. Cox regression model was analyzed by multivariate analysis. The stage of UICC lymph node metastasis was correlated with prognosis (X2 = 4.85, P = 0.028). Conclusion The expression of STAT1 protein in gastric cancer is negatively correlated with the expression of Survivin protein. Both have nothing to do with survival. STAT1 is a molecular marker predicting advanced gastric cancer. Survivin is a molecular marker of gastric cancer lymph node metastasis. UICC gastric cancer lymph node staging is an important prognostic indicator.
