腺苷预处理对婴幼儿法洛四联症根治术中肌钙蛋白Ⅰ和肌酸磷酸激酶同工酶的影响

目的探讨腺苷预处理对婴幼儿法洛四联症(TOF)根治术中肌钙蛋白Ⅰ(cTnI)和肌酸磷酸激酶同工酶(CK-MB)的影响。方法将120例体外循环下行TOF根治术的婴幼儿随机分为治疗组60例和对照组60例,治疗组在转流前应用腺苷,观察两组患儿恢复窦性心律的时间和多巴胺的平均用量,另分别于转流前、开放循环后30min和术后12h测定血清cTnI、CK-MB水平并进行比较。结果两组患儿的多巴胺用量间差异有统计学意义(P<0.01)。腺苷组开放循环后30min、术后12h的血清cTnI、CK-MB水平分别与对照组开放循环后30min、术后12h比较,差异有统计学意义(P<0.01)。结论腺苷预处理可以降低行TOF根治术患儿血清cTnI和CK-MB水平,对心肌具有良好的保护作用。 Objective To investigate the effects of adenosine preconditioning on cTnI and CK-MB in infants with tetralogy of Fallot (TOF). Methods A total of 120 infants with TOF underwent cardiopulmonary bypass were randomly divided into treatment group (60 cases) and control group (60 cases). In the treatment group, adenosine was administered before the bypass, the duration of sinus rhythm and dopamine The mean levels of cTnI and CK-MB in serum were also measured at 30 min before open circulation, 30 min after open circulation, and 12 h after operation respectively. Results There was a significant difference in the amount of dopamine between the two groups (P <0.01). The levels of cTnI and CK-MB in serum of 30 days after open circulation of adenosine group and 12 hours after operation were significantly different from those of 30 minutes and 12 hours after open circulation in control group (P <0.01). Conclusions Adenosine preconditioning can reduce the level of cTnI and CK-MB in children with TOF and has a good protective effect on myocardium.
