目的探讨小脑幕脑膜瘤手术入路、显微切除方法以及如何最大限度切除肿瘤和最大程度保留神经功能。方法分析1995年1月~2005年12月间56例小脑幕脑膜瘤的病例资料。经临床、CT、磁共振成像(MR I)、数字减影血管造影(DSA)诊断后,进行显微操作切除肿瘤。结果达S impsonⅠ级全切除12例(21.4%),S impsonⅡ级全切除32例(57.1%),肿瘤次全切除10例(17.9%),大部分切除2例(3.5%),术中无死亡。结论利用现代影像技术,早期诊断、规范分类、选择合适的手术入路,应用显微神经外科技术,可以明显提高本病的治疗效果。
Objective To investigate the surgical approaches to the cerebellar meningioma, the methods of microdissection, and how to remove the tumor to the maximum extent and preserve the neurological function to the maximum extent. Methods From January 1995 to December 2005 56 cases of cerebellar meningioma cases. After clinical, CT, magnetic resonance imaging (MRI), digital subtraction angiography (DSA) diagnosis, microsurgical resection of the tumor. Results The total resection rate was 17.4% in S impson class Ⅰ, 32 cases (57.1%) in S impson Ⅱ grade, 10 cases (17.9%) in subtotal resection, and 2 cases (3.5% death. Conclusion The use of modern imaging techniques, early diagnosis, standardized classification, select the appropriate surgical approach, the application of micro-neurosurgery technology, can significantly improve the treatment of this disease.
