心肺复苏人工-机械转换所致胸外按压暂停时长的相关因素分析

目的:分析心肺复苏人工-机械转换所致胸外按压暂停时长的相关因素。方法:本研究为回顾性队列研究,主要研究人群为2019年1月至2020年12月期间在湖州市第一人民医院急诊医学科就诊的符合纳入排除标准且接受机械心肺复苏的院外心脏骤停患者;收集患者救治过程中的基本资料、救治相关资料、人工-机械按压转换数据资料。多重线性回归分析患者基本资料、救治相关资料与人工-机械按压转换过程中的按压暂停时长(简称按压暂停时长)的相关性,同时分析当班护士心肺复苏救治资质对心肺复苏按压质量的影响。结果:符合纳入排除标准的患者32例,患者身高、实际体重、体重指数与按压暂停时长呈线性正相关(n r=0.61、0.92、0.49;n P<0.01)。经过多重回归分析发现,患者实际体重是患者按压暂停时长过长的独立危险因素(n P <0.01)。对当班护士心肺复苏资质培训情况进行分析发现,高级心血管生命支持资质组患者按压分数明显高于无资质组患者( n χ2=0.002,n P<0.01)。n 结论:患者实际体重、当班护士心肺复苏资质培训情况与心肺复苏人工-机械转换所致胸外按压暂停时长具有一定相关性,值得进一步研究。“,”Objective:To analyze the factors related to the duration of chest compression pause time during the manual-mechanical conversion process in cardiopulmonary resuscitation (CRP).Methods:A retrospective study was devised in a cohort comprising patients with out-of-hospital cardiac arrest, who were assigned to receive mechanical CRP in the Department of Emergency of Huzhou First People\'s Hospital from January 2019 to December 2020. Patient’s general characteristics, CRP data and data on CRP-free intervals were collected multiple linear regression to analyze associations with the duration of chest compression pause time during the manual-mechanical conversion process in CRP. At the same time, the effect of CPR treatment qualification of nurses on CPR compression quality was evaluated.Results:The study selected 32 eligible patients. Patient\'s height, actual body weight, and body mass index showed a positive liner correlation with the duration of chest compression pause time (n r=0.61, 0.92, 0.49; n P<0.01). Multiple stepwise regression analysis showed that actual body weight was an independent risk factor for prolonged duration of chest compression pause time (n P<0.01). Moreover, responsible nurses with advanced cardiac life support (ACLS) certification had significantly higher compression scores than those without ACLS certification (χn 2=0.002, n P<0.01).n Conclusions:The actual body weight of patients and the ACLS qualification of nurses on duty have a certain relationship with the duration of chest compression pause time during the manual-mechanical conversion process in CRP, which is worthy of further research.
