目的探讨N-端脑利钠肽前体(NT-pro BNP)对子痫前期孕妇心功能的评价效果。方法选取收治的重度子痫前期孕妇120例、轻度子痫前期孕妇120例、正常妊娠孕妇120例于门诊第1天和终止妊娠前行NT-pro BNP检测及心功能指标检测。结果轻度子痫前期孕妇与正常妊娠孕妇相比,NT-pro BNP、肌酸激酶同工酶、肌红蛋白、肌钙蛋白、平均动脉压、心率明显增加;重度子痫前期孕妇与正常妊娠孕妇、轻度子痫前期孕妇相比,NT-pro BNP、肌酸激酶同工酶、肌红蛋白、肌钙蛋白、平均动脉压、心率明显增加,而射血分数明显下降。随着妊娠期高血压病情的出现及加重,心功能逐渐减弱,妊娠并发症(心功能不全、肝损害、肾损害、眼底改变、胎盘早剥)发生率逐渐增加;随着NT-pro BNP水平升高,肌酸激酶同工酶、肌红蛋白、肌钙蛋白逐渐升高,心功能逐渐下降,妊娠并发症(心功能不全、肝损害、肾损害、眼底改变、胎盘早剥)发生率逐渐增加,差异均有统计学意义(P<0.05)。结论 NT-pro BNP随子痫前期病情加重而升高,能影响孕妇心功能,增加妊娠并发症的发生风险;NT-pro BNP可用于子痫前期孕妇心功能的评价。
Objective To investigate the effect of N-terminal pro-brain natriuretic peptide (NT-pro BNP) on cardiac function in pregnant women with preeclampsia. Methods 120 cases of severe preeclampsia, 120 cases of mild preeclampsia and 120 cases of normal pregnant women were enrolled in this study. NT-pro BNP test and cardiac function test were performed on the first day of outpatient clinic and the termination of pregnancy. Results Compared with normal pregnant women, the levels of NT-pro BNP, myoglobin, myoglobin, troponin, mean arterial pressure and heart rate in pregnant women with mild preeclampsia were significantly increased. Compared with normal pregnant women NT-pro BNP, creatine kinase MB, myoglobin, troponin, mean arterial pressure and heart rate were significantly increased in pregnant women and mild preeclampsia, while the ejection fraction was significantly decreased. The incidence of pregnancy complications (heart failure, liver damage, kidney damage, fundus changes, placental abruption) gradually increased with the emergence and aggravation of pregnancy-induced hypertension. As the level of NT-pro BNP Increased creatine kinase isoenzyme, myoglobin, troponin gradually increased cardiac function gradually decreased, pregnancy complications (cardiac insufficiency, liver damage, renal damage, fundus changes, the incidence of placental abruption) gradually Increase, the difference was statistically significant (P <0.05). Conclusion NT-pro BNP increases with the severity of preeclampsia, which may affect the cardiac function of pregnant women and increase the risk of pregnancy complications. NT-pro BNP can be used to evaluate the cardiac function of pregnant women with preeclampsia.
