目的探讨2型糖尿病患者血清糖类抗原CA19-9水平的变化及意义,并对相关影响因素进行分析。方法采用化学发光法检测254例糖尿病患者和200例健康体检者血清CA19-9水平,同时对糖尿病相关因素与血清CA19-9浓度的相关性进行分析。结果糖尿病组血清CA19-9水平显著高于正常对照组(t=19.170,P=0.000),两组间BMI、HDLC、GFR差异均无统计学意义(P﹥0.05)。DM组TC、TG、LDLC、FPG、HbA1c、mAlb、CA19-9等均显著高于对照组(P﹤0.01),而FCP低于对照组(P﹤0.05)。单因素分析显示,DM患者血清CA19-9水平与病程、malb、FCP及HbA1c相关(P﹤0.05),其中与HbA1c密切相关(r=0.192,P=0.008),而与年龄、BMI、TC、TG、HDLC、LDLC、FPG、GFR、FINS、FCP等均不相关。结论 2型DM患者血清CA19-9低水平升高,且与HbA1c密切相关,动态观察血糖控制前后CA19-9水平变化对糖尿病的预后具有一定的意义。
Objective To investigate the changes and significance of serum carbohydrate antigen CA19-9 in patients with type 2 diabetes mellitus and to analyze the related factors. Methods Chemiluminescence was used to detect serum CA19-9 levels in 254 diabetics and 200 healthy controls, and the correlation between diabetes-related factors and serum CA19-9 concentration was analyzed. Results The level of serum CA19-9 in diabetic group was significantly higher than that in normal control group (t = 19.170, P = 0.000). There was no significant difference in BMI, HDLC and GFR between the two groups (P> 0.05). The levels of TC, TG, LDLC, FPG, HbA1c, mAlb and CA19-9 in DM group were significantly higher than those in control group (P <0.01), while FCP was lower than that in control group (P <0.05). Univariate analysis showed that the level of serum CA19-9 in DM patients was correlated with the duration of disease, malb, FCP and HbA1c (P <0.05), and was closely related to HbA1c (r = 0.192, P = 0.008) TG, HDLC, LDLC, FPG, GFR, FINS, FCP, etc. are not related. Conclusions Serum CA19-9 level is elevated in patients with type 2 diabetes mellitus and is closely related to HbA1c. To observe the changes of CA19-9 level before and after blood glucose control has some significance on the prognosis of diabetes mellitus.
