目的 着重分析和讨论用现代囊外白内障摘除术(modernextracapsularcataractextrac tion ,ECCE)方法行后房型人工晶体(intraocularlens ,IOL)植入术发生角膜水肿及角膜失代偿的原因。方法 本组病例来源于4 0 8例4 35眼ECCE后房型IOL植入术所发生并发症。结果 本组4 0 8例4 35眼中发生角膜水肿及失代偿4 3眼,其中角膜水肿4 2眼,角膜失代偿1眼。结论 作者认为后房型IOL植入术所发生角膜水肿及角膜失代偿与下面因素有关:①与眼内灌注液有关,②与患者的年龄有关,③但更主要与医生的操作关系密切,手术操作时间长,机械性对角膜内皮损伤,导致角膜内皮部分功能丧失,最终发生角膜水肿及失代偿,因此提高医生的手术的熟练程度是减少此并发症的关键
Objective To analyze and discuss the causes of corneal edema and corneal decompensation caused by intraocular lens implantation by modern extracapsular cataract extraction (ECCE). Methods This group of patients from 408 cases of 43 cases of ECCE posterior chamber IOL implantation complications occurred. Results There were 43 corneal edema and decompensation in 480 eyes of 458 patients in this group, including 42 corneal edema and 1 corneal decompensation. Conclusions The authors believe that corneal edema and corneal decompensation associated with posterior chamber IOL implantation are related to intraocular perfusion fluid, ② to patient’s age, and ③ more important to physician’s operation. Surgery Long operating time, mechanical damage to the corneal endothelium, resulting in loss of corneal endothelial function, the final occurrence of corneal edema and decompensation, thus improving the proficiency of the doctor’s surgery is the key to reducing this complication
