目的 探讨老年重症下呼吸道感染患者甲状腺激素水平的改变及与病情和预后的关系 ,为诊治此类疾病提供依据。方法 用放射免疫法测定 30例老年重症下呼吸道感染患者 (感染组 )及对照组血浆三碘甲状腺原氨酸 (T3 )、甲状腺素 (T4)及促甲状腺素 (TSF)水平。结果 与对照组比较老年重症下呼吸道感染组T3 、T4均明显降低 ,TSH两组无差异。感染组中死亡 13例 ,其T3 、T4水平明显低于该组中存活者。结论 老年重症下呼吸道感染患者中存在正常甲状腺功能病态综合征 ,低T3 、T4综合症可作为预后不良的指标。
Objective To investigate the changes of thyroid hormone levels in elderly patients with severe lower respiratory tract infection and its relationship with the disease and prognosis, and to provide basis for the diagnosis and treatment of such diseases. Methods Plasma levels of triiodothyronine (T3), thyroxine (T4) and thyroid stimulating hormone (TSF) in 30 elderly patients with severe lower respiratory tract infection (infection group) and in control group were determined by radioimmunoassay. Results Compared with the control group, the T3 and T4 of elderly patients with severe lower respiratory tract infection were significantly lower, while there was no difference between the two groups. Thirteen patients died in the infected group, and the levels of T3 and T4 in the infected group were significantly lower than those in the infected group. Conclusion There are normal thyroid functional syndromes in elderly patients with severe lower respiratory tract infection. Low T3 and T4 syndromes may be used as indicators of poor prognosis.
