目的 探讨2型糖尿病并发肺结核(type 2 diabetes mellitus complicated with pulmonary tuberculosis,T2DMTB)患者血清中炎症细胞因子Toll样受体4(toll-like receptor 4,TLR4)和脂多糖受体14(lipopolysaccharide receptor 14,LPS受体14;又称CD14)水平的变化.方法 选取2014年9月至2015年6月在黑龙江省传染病防治院就诊的T2DMTB患者40例(T2DMTB组),2型糖尿病(type 2 diabetes mellitus,T2DM)患者40例(T2DM组),以及体检科筛查出的健康者40名(对照组)作为研究对象.收集研究对象的人口学特征资料,并抽取清晨空腹静脉血3~4 ml.采用酶联免疫吸附试验检测研究对象血清中TLR4和CD14的水平,并进行比较.结果 对照组、T2DM组和T2DMTB组血清中TLR4水平分别为(10.21±2.29) ng/ml、(19.40±1.80) ng/ml、(15.27±2.52) ng/ml,三组间比较差异有统计学意义(F=76.33,P<0.01);进一步的两两比较显示,T2DM组和T2DMTB组均高于对照组,T2DMTB组低于T2DM组,差异均有统计学意义(P值均<0.01).对照组、T2DM组和T2DMTB组血清中CD14水平分别为(9.17±1.62) ng/ml、(16.48±1.54) ng/ml、(10.86±1.65) ng/ml,三组间比较差异有统计学意义(F=113.92,P<0.01);进一步的两两比较显示,T2DM组和T2DMTB组均高于对照组,T2DMTB组低于T2DM组,差异均有统计学意义(P值均<0.01).结论 T2DMTB组血清中TLR4和CD14水平高于对照组,而低于T2DM组;T2DMTB患者血清中TLR4和CD14水平变化,对T2DMTB的发病机制有提示作用.“,”Objective To investigate the relationship between serum levels of inflammnatory cytokines Toll Like receptor 4 (TLR4),lipopolysaccharide receptor 14 (LPS receptor) and type 2 diabetes mellitus complicated with tuberculosis (T2DMTB).Methods We recruited 40 cases with T2DMTB and type 2 diabetes mellitus (T2DM) respectively,and 40 cases of healthy people from the Heilongjiang province fourth hospital between September 2014 and June 2015.The demographic characteristics of the subjects were collected.In addition,3 to 4 ml venous blood was collected in the early morning.The serum levels of TLR4 and CD14 were measured by enzyme-linked immunosorbent assay (ELISA) and compared.Results The serum grades of TLR4 in the control group,T2DM group and T2DMTB group were (10.21 ± 2.29) ng/ml,(19.40 ± 1.80) ng/ml and (15.27 ± 2.52) ng/ml,respectively.The differences among the three groups were significant (F=76.33,P<0.01).The serum grades of CD14 in the control group,T2DM group and T2DMTB group were (9.17±1.62) ng/ml,(16.48± 1.54) ng/ml and (10.86±1.65) ng/ml,respectively.The differences among the three groups were significant (F=113.92,P<0.01).The serum grades of TLR4 and CD14 in T2DM and T2DMTB groups were significantly higher than healthy control group,but the serum levels of TLR4 and CD14 in T2DMTB group was significantly by com pared with T2DM group (P<0.05).Conclusion The serum levels of TLR4 and CD14 in T2DMTB group were higher than those in control group,but lower than that in T2DM group.The changes of the serum levels of TLR4 and CD14 with T2DMTB suggest that they play an important prompt role on the pathogenesis of T2DMTB.
