目的探讨超声弹性成像评分法和比值法在乳腺非肿块型病变良恶性鉴别诊断中的价值。方法经手术病理证实的39例乳腺非肿块型病变的超声弹性成像图像进行回顾性分析,采用评分法和比值法评估病变区,用ROC曲线和诊断试验评价指标来评估各自的诊断价值。结果评分法和比值法诊断乳腺非肿块型病变良恶性的ROC曲线下面积(Az)分别为0.895(P=0.00)和0.943(P=0.00),其灵敏度、特异度、准确度分别为:69.6%、93.8%、79.5%和91.3%、93.8%、95.5%;其中,评分法中评分为3分的病变,与应用比值法之间比较,比值法的准确度提高了35.7%。结论超声弹性成像比值法灵敏度和特异度均较高,而且更加客观,明显提高了超声诊断的准确度。
Objective To investigate the value of ultrasound elastography scoring method and ratio method in the differential diagnosis of benign and malignant breast lesions. Methods Thirty-nine cases of non-bulging breast lesions confirmed by surgery and pathology were retrospectively analyzed by echocardiography. The lesions were evaluated by the score method and the ratio method. The diagnostic value was evaluated by ROC curves and diagnostic tests. Results The area under the receiver operating characteristic (ROC) curve for the diagnosis of benign and malignant breast lesions was 0.895 (P = 0.00) and 0.943 (P = 0.00), respectively. The sensitivity, specificity and accuracy were 69.6 %, 93.8%, 79.5% and 91.3%, 93.8% and 95.5%, respectively. Among them, the score with 3 points in the score method was 35.7% more accurate than the application ratio method. Conclusion The sensitivity and specificity of the ultrasound elastography ratio method are higher, more objective, and significantly improve the accuracy of ultrasonic diagnosis.
