2015年西平县全人群死因监测漏报调查

目的了解西平县全人群死因监测工作漏报情况及漏报的主要原因,以改进和完善死因登记报告工作,准确估计西平县全人群死亡率。方法按照2012-2014年3年粗死亡率从高到低排序平均分为3个层次,分别从高、中、低每个层次中选取死亡率中位数的二郎乡、五沟营镇、杨庄乡作为被调查乡镇;在被调查乡镇中按照3年平均人口数大小顺序平均分为3组,分别选取大、中、小人口分组中位数所在行政村共9个行政村作为调查对象。调查员在村干部、村医协助下,通过村民小组负责人根据户主信息对每户的2012-2014年死亡情况进行逐一回忆、填写,并通过户籍、计生、民政、妇幼等补充核实死亡信息,确定死亡摸底名单;将死亡者名单与监测系统常规报告的死亡名单进行自动与人工比对,经再次调查核实,统计漏报率。结果 3年平均漏报率为8.17%(47/575),不同乡镇间漏报率差别较大,在被调查乡镇中二郎乡漏报率仅为2.55%(6/235),而五沟营镇及杨庄乡漏报率分别为11.33%(23/203)、13.14%(1 8/137)。结论西平县全人群死亡漏报情况仍较为严重,不同乡镇间漏报率差别较大,应进一步健全组织完善机制,加强各级疾病预防控制人员责任心培养,规范死因监测报告与管理质量。 Objective To understand the causes of omission in monitoring the cause of death of the entire population in Xiping County and the main reasons for the omission, in order to improve and perfect the cause of death registration report and accurately estimate the mortality rate of the entire population in Xiping County. With the help of village cadres and village doctors, the investigators recall each household’s deaths in 2012-2014 according to the head of household information and fill out the information and verify the death information through household registration, family planning, civil affairs, women and children, Determine the deathmatch list; compare the list of dead and the list of deaths routinely reported by the monitoring system for automatic and manual comparison, and after further investigation and verification, calculate the false negative rate. Results The average 3-year false negative rate was 8.17% (47/575). There was a big difference between different townships and omissions in reporting rate, which was only 2.55% (6/235) in Zhongjirong Township, The misleading rates of the town and Yangzhuang Township were 11.33% (23/203) and 13.14% (18/137) respectively. Conclusion The death and omission of the entire population in Xiping County is still serious, and the rate of omission in different villages and towns is quite different. Organizational mechanisms should be further perfected to enhance the sense of responsibility training of disease prevention and control personnel at all levels, and to standardize the cause of death monitoring report and management quality.
