目的探讨维吾尔族、汉族原发性高血压患者维生素D受体(VDR)和肾素mRNA表达及差异性。方法2013年2月至2013年6月,入选新疆医科大学第一附属医院心脏中心高血压科门诊原发性高血压(EH)患者90例,包括维吾尔族44例,汉族46例(EH组)。同时入选108名体检中心体检正常的人群,包括维吾尔族53人,汉族55人(对照组)。检测血清25羟维生素D[25(OH)D]和肾素等指标;同时采用实时荧光定量聚合酶链反应(RT-PCR)检测外周血中VDR和肾素mRNA表达水平。结果与维吾尔族、汉族对照组比较,维吾尔族、汉族EH组VDR mRNA表达较低[维吾尔族:(0.61±0.16)比(0.91±0.23);汉:(0.97±0.12)比(1.83±0.26)],肾素mRNA表达较高[维吾尔族:(2.66±0.17)比(1.83±0.21);汉:(1.79±0.23)比(1.06±0.18),均P<0.05]。与汉族EH组比较,维吾尔族EH组VDR mRNA表达较低,而肾素mRNA表达较高(均P<0.05)。相关分析结果显示:VDR mRNA表达与25(OH)D呈正相关,与体质量指数、肾素、收缩压、舒张压和肾素mRNA表达呈负相关(均P<0.05)。Logistic回归分析显示:民族、体质量指数、25(OH)D、肾素和血管紧张素Ⅱ为高血压的影响因素(均P<0.05)。结论维吾尔族、汉族EH患者VDR mRNA表达下降,肾素mRNA升高。维吾尔族VDR mRNA表达较汉族更低,肾素mRNA表达较高。VDR和肾素mRNA表达在维吾尔族、汉族存在差异。
Objective To investigate the expression and difference of vitamin D receptor (VDR) and renin mRNA in Uygur and Han patients with essential hypertension. Methods From February 2013 to June 2013, 90 patients with essential hypertension (EH) from the Outpatient Hypertension Clinic in the First Affiliated Hospital of Xinjiang Medical University were enrolled. Among them, 44 were Uygur, 46 were Han (EH) . At the same time selected 108 physical examination center of the normal physical examination, including 53 Uighurs, Han 55 (control group). Serum 25 Hydroxyvitamin D [25 (OH) D] and renin were detected. The levels of VDR and Renin mRNA in peripheral blood were detected by real-time fluorescent quantitative polymerase chain reaction (RT-PCR). Results Compared with Uygur and Han controls, VDR mRNA expression was lower in Uighur and Han EH patients (Uighur: 0.61 ± 0.16 vs. 0.91 ± 0.23; Han: 0.97 ± 0.12 vs 1.83 ± 0.26, (2.66 ± 0.17) vs (1.83 ± 0.21); Han: (1.79 ± 0.23) vs (1.06 ± 0.18), both P <0.05]. Compared with the Han EH group, VDR mRNA expression was lower in Uighur EH patients and higher in renin mRNA (all P <0.05). Correlation analysis showed that VDR mRNA expression was positively correlated with 25 (OH) D and negatively correlated with body mass index, renin, systolic pressure, diastolic pressure and mRNA expression of renin (all P <0.05). Logistic regression analysis showed that nationality, body mass index, 25 (OH) D, renin and angiotensin Ⅱ were the influencing factors of hypertension (all P <0.05). Conclusion The VDR mRNA expression and the renin mRNA in Uighur and Han patients with EH are decreased. Uygur VDR mRNA expression lower than the Han, Renin mRNA expression was higher. VDR and renin mRNA expression in Uygur and Han differences.
