目的:探讨隐性骶椎裂对尿道外括约肌功能的影响及尿道外括约肌肌电图(EMG)检查对神经源性膀胱的临床意义。方法:30例经放射学证实隐性骶椎裂患者进行尿道外括约肌定量EMG测定。结果:尿道外括约肌均呈轻、重不同的神经源性损害EMG,表现为运动单位电位平均时限增宽,多相电位百分率增多,部份记录到自发性电位、束颤电位、巨大电位以及在尿道外括约肌记录的骶神经反射潜伏时延长。结论:马尾神经损害是致尿道外括约肌功能异常的直接原因,尿道外括约肌EMG检查能明显提高隐性骶椎裂的亚临床阳性率,有助于神经源性膀胱患者的早期诊断,EMG是评价骶神经与尿道外括约肌功能的有效方法。
Objective: To investigate the effect of occult sacral fissure on the function of the external urethral sphincter and the clinical significance of the EMG examination of the urethral sphincter to the neurogenic bladder. Methods: Quantitative EMG measurements of the external urethral sphincter were performed in 30 patients with recurrent sacral fissure confirmed by radiology. Results: The urethral sphincter showed mild and severe neurogenic damage to EMG. The mean duration of motor unit potentials was widened and the percentage of multiphase potentials increased. Some of them recorded spontaneous potentials, bundle fibrillation potentials, huge potentials, The sacral reflex recorded by the urethral sphincter is prolonged. Conclusion: The cauda equina nerve injury is the direct cause of dysfunction of the external urethral sphincter. EMG examination of the external urethral sphincter can significantly improve the subclinical positive rate of cryptogenic sacrococcygeal fractures, which is helpful for the early diagnosis of neurogenic bladder.
