目的:探讨冲洗液温度对行经皮肾镜取石术患者的临床效果以及选择素P和E的影响。方法:选择2015年5月至2017年5月本院收治的经皮肾镜取石手术患者60例为研究对象,随机分为室温组和温控组,每组30例。室温组患者术中及术后24 h室温为25℃,术中灌洗液不加温;温控组患者术中及术后24 h室温为25℃,术中灌洗液恒温加热至37℃。观察两组患者的临床指标及术前、术中和术后的体温变化和对应时间的D-二聚体、选择素P和选择素E的变化以及不良反应发生率。结果:温控组患者的术中出血量、手术时间和住院时间都明显优于室温组(n P<0.05);温控组患者术中和术后的体温明显高于室温组(n P<0.05);温控组患者术中和术后的D-二聚体、选择素P和选择素E的含量都低于室温组(n P<0.05);温控组患者的不良反应发生率明显低于室温组(n P<0.05)。n 结论:在对患者进行经皮肾镜手术的过程当中使用接近体温的冲洗液可以稳定患者的体温,减少不良反应发生。“,”Objective:To investigate the clinical effect of irrigation fluid temperature on percutaneous nephrolithotomy and the effect of selectin P and E.Methods:From May 2015 to May 2017, 60 patients with percutaneous nephrolithotomy were selected as the study subjects in our hospital.They were randomly divided into room temperature group and temperature control group with 30 patients in each group.In the room temperature group, the room temperature was 25 degrees 24 hours after operation, and the lavage fluid was not heated during operation.In the temperature control group, room temperature was 25 degrees 24 hours after operation, and lavage fluid was heated 37 degrees at constant temperature during operation.The clinical indicators, changes of body temperature before, during and after operation, changes of D-dimer, selectin P and selectin E at corresponding time, and the incidence of adverse reactions were observed.Results:The blood loss, operation time and hospitalization time in the temperature control group were significantly better than those in the room temperature group.The body temperature in the temperature control group was significantly higher than that in the room temperature group during and after operation.The levels of D-dimer, selectin P and selectin E in intraoperative and postoperative temperature control group were lower than those in room temperature group.The incidence of adverse reactions in the temperature control group was significantly lower than that in the room temperature group, the difference was statistically significant(n P<0.05).n Conclusions:In the process of percutaneous nephroscopic surgery, the use of fluids close to body temperature can stabilize the patient's body temperature and reduce the occurrence of adverse reactions.
