目的探讨声触诊组织定量分析(VTQ)技术评价儿童原发性肾病综合征(NS)肾间质纤维化程度的应用价值。方法将150例NS患儿肾间质纤维化程度分为轻、中、重度三组,50例正常儿童作为对照组,利用VTQ检测肾实质剪切波速(SWS),将检测结果进行统计学分析。结果轻度肾间质纤维化组与对照组比较SWS差异无统计学意义(P=0.173),其他两组与对照组比较及各组间两两比较SWS差异也均有统计学意义(P<0.05)。VTQ诊断NS患儿中、重度肾间质纤维化的ROC曲线下面积分别为0.93、0.96,最佳截断点分别为2.84m/s、3.02m/s,灵敏度分别为89.5%、92.9%,特异度分别为91.3%、88.2%。结论 VTQ技术可无创性定量判断NS患儿的肾间质纤维化程度。
Objective To investigate the value of quantitative palpation (VTQ) in evaluating the degree of renal interstitial fibrosis in children with primary nephrotic syndrome (NS). Methods 150 NS children with renal interstitial fibrosis were divided into mild, moderate and severe three groups, 50 normal children as control group, using VTQ detection of renal parenchymal shear wave velocity (SWS), the test results were statistically analyzed . Results Compared with the control group, there was no significant difference in SWS between the mild renal interstitial fibrosis group and the control group (P = 0.173), the other two groups compared with the control group and the SWS differences between the two groups were also statistically significant (P < 0.05). The area under the ROC curve of VTQ diagnosis of severe renal interstitial fibrosis was 0.93,0.96, the best cutoff points were 2.84m / s, 3.02m / s, the sensitivity was 89.5%, 92.9%, respectively Degrees were 91.3%, 88.2%. Conclusion The VTQ technique can be used to evaluate the degree of renal interstitial fibrosis in children with NS noninvasively and quantitatively.
