目的:分析2019年8月北京市朝阳区确诊的l例人感染H5N6禽流感病例流行病学特征,为人感染H5N6禽流感疫情防控提供依据。方法:对病例家属进行访谈,获取病例发病、诊治过程及密切接触者信息,采集病例及密切接触者的呼吸道标本、病家及共同暴露者家中剩余禽类标本进行核酸检测及全基因组分析。结果:病例表现为重症肺炎,发病前仅加工制作邻居从湖南某市场购买的冷冻鸡。病例发病早期采集的肺泡灌洗液、痰液为H5N6禽流感病毒核酸阳性,肺泡灌洗液与病例家中剩余冷冻禽类制品全基因组序列分析高度同源,HA基因Clade分型属于高致病性禽流感病毒2.3.4.4分支。结论:加工制作带毒的冷冻禽制品也可感染禽流感病毒,提示未经检疫的冷冻禽制品存在风险。“,”Objective:To analyze the epidemiological characteristics of the first human case of avian influenza A(H5N6) virus infection in Beijing, so as to provide evidences for prevention and control of avian influenza A(H5N6) virus infection.Methods:The relatives of the case were interviewed to obtain data of disease onset, treatment process and information of close contacts. The respiratory tract samples of the case and the close contacts, poultry specimens frozen at homes of the case and persons of co-exposure were collected for nucleic acid testing and genome analysis.Results:The case had severe pneumonia. Before onset, the patient solely cooked the frozen chicken bought by the neighbour from one market in Human province. The samples of bronchoalveolar lavage fluid, sputum collected at the early stage of diseases were positive for the nucleic acid of avian influenza A(H5N6) virus. Genomic analysis indicated that the sequences obtained from bronchoalveolar lavage fluid and frozen poultry samples were highly similar. The Clade typing of HA gene showed the virus belonged to highly pathogenic avian influenza virus clade 2.3.4.4.Conclusions:Processing of frozen poultry products may also cause infection, suggesting that process of unquarantined frozen poultry products can also be risky.
