目的探讨早发性和晚发性帕金森病(PD)患者临床症状的研究。方法连续收集2014年7月—2016年7月就诊于河北医科大学第一医院神经内科的73例PD患者,根据起病年龄分为早发组(EOP组,起病年龄≤50岁)和晚发组(LOP组,起病年龄>50岁),比较二组的一般资料及临床症状。结果①73例PD患者中男31例,女42例;平均发病年龄(60.5±9.97)岁。②EOP组共14例,平均发病年龄47(42.75~50)岁,LOP组共59例,平均发病年龄63(58~69)岁。③EOP组病程、MMSE评分、MoCA评分及UPDRSⅢ评分高于LOP组(P<0.05),LOP组吸烟及嗅觉障碍的发生率高于EOP组(P<0.05)。结论早发性PD患者病情进展速度比晚发者缓慢,但运动症状及认知障碍的严重程度比晚发者严重。吸烟可能延缓PD的发生,晚发的PD患者嗅觉障碍的发病率更高。
Objective To investigate the clinical symptoms of patients with early-onset and late-onset Parkinson’s disease (PD). Methods Seventy-three patients with PD who were admitted to Department of Neurology, First Hospital of Hebei Medical University from July 2014 to July 2016 were divided into two groups according to the onset age: early onset group (EOP group, onset age ≤50 years) and late (LOP group, onset age> 50 years), general information and clinical symptoms of the two groups were compared. Results ①73 cases of PD patients, 31 males and 42 females; average age of onset (60.5 ± 9.97) years. ② There were 14 cases in the EOP group with an average age of onset of 47 (42.75-50) years. There were 59 cases in the LOP group with an average age of onset of 63 (58-69) years. (3) The course of EOP group, MMSE score, MoCA score and UPDRS III score were higher than LOP group (P <0.05). The incidence of smoking and olfactory dysfunction in LOP group was higher than EOP group (P <0.05). Conclusions The progression of the disease in patients with early-onset PD is slower than that in late-onset PD patients. However, the severity of motor symptoms and cognitive impairment are more serious than those in late-stage PD patients. Smoking may delay the onset of PD, and the incidence of olfactory dysfunction in late-onset PD patients is higher.
