目的总结骨肉瘤的影像表现,探讨其影像特征。方法经穿刺或手术病理证实的35例骨肉瘤,所有病例均进行了X线检查,同时有25例进行CT或MR检查,8例进行MR增强检查。影像学表现并与病理学对照进行分析。结果股骨远端17例与胫骨近端8例为骨肉瘤好发部位。其特征性影像表现为骨质破坏、肿瘤骨、骨膜反应及软组织肿块,部分病例侵犯骺板及骨骺,甚至突破关节面或向关节对侧骨质跳跃转移。MR T1WI增强检查表现为早期肿瘤组织边缘强化而中心充盈延迟。CT可显示筛孔征及细小的肿瘤骨。结论骨肉瘤具有一定的影像特征;X线是骨肉瘤的首选检查方法,CT和MR可为临床提供更为全面的影像学信息。
Objective To summarize the imaging manifestations of osteosarcoma and explore its imaging features. Methods Thirty-five cases of osteosarcoma proved by puncture or pathology were examined by X-ray. All the cases were examined by CT or MR in 25 cases and MR enhanced in 8 cases. Imaging findings were analyzed with pathology controls. Results 17 cases of distal femur and 8 cases of proximal tibia osteosarcoma prone site. The characteristic images showed bone destruction, tumor bone, periosteal reaction and soft tissue mass, some cases of invasion of epiphyseal plate and epiphysis, and even break through the articular surface or to the contralateral bone jump jump. MR T1WI enhanced examination showed enhanced edge of early tumor tissue and central filling delay. CT can show signs of mesh and small tumor bone. Conclusion Osteosarcoma has some imaging features. X-ray is the first choice of osteosarcoma. CT and MR can provide more comprehensive imaging information for clinic.
