目的探讨血液检验红细胞参数在贫血患者中的鉴别诊断价值。方法随机选取2015年3月至2016年3月沈阳市皇姑区中心医院收治的贫血患者204例为研究对象,按照不同病因分为A组(缺铁性贫血,n=118)、B组(地中海贫血,n=72)、C组(再生障碍性贫血,n=14)。以同期到本院体检的健康人100例作为D组。分析所有受检者血液中红细胞计数、红细胞体积分布宽度、红细胞平均体积、红细胞平均血红蛋白量、血红蛋白含量等红细胞参数的变化。结果A、B、C组患者红细胞计数及血红蛋白含量均明显低于D组,差异有统计学意义(P<0.05)。与D组比较,A组患者红细胞平均体积、红细胞平均血红蛋白量明显下降,差异有统计学意义(P<0.05),但红细胞体积分布宽度明显升高,差异有统计学意义(P<0.05);B组患者红细胞平均体积、红细胞平均血红蛋白量明显下降,差异有统计学意义(P<0.05),但红细胞体积分布宽度差异无统计学意义(P>0.05);C组患者红细胞平均体积明显增加,差异有统计学意义(P<0.05),但红细胞平均血红蛋白量、红细胞体积分布宽度差异无统计学意义(P>0.05)。结论在贫血疾病的诊断中,血液检验红细胞参数能对不同类型的贫血进行鉴别,值得深入研究与推广。
Objective To investigate the differential diagnostic value of blood test erythrocyte parameters in patients with anemia. Methods A total of 204 patients with anemia admitted from March 2015 to March 2016 in Huanggu District Central Hospital were randomly divided into A group (iron deficiency anemia, n = 118), group B Thalassemia, n = 72), group C (aplastic anemia, n = 14). In the same period to our hospital physical examination of 100 healthy people as D group. The changes of erythrocyte parameters such as erythrocyte count, red blood cell volume distribution width, average volume of red blood cells, average erythrocyte hemoglobin, and hemoglobin in all subjects were analyzed. Results The erythrocyte count and hemoglobin in group A, B and C were significantly lower than those in group D (P <0.05). Compared with group D, the average volume of red blood cells and the average hemoglobin of erythrocytes in group A were significantly decreased (P <0.05), but the width of erythrocytes was significantly increased (P <0.05). The mean volume of erythrocytes and mean hemoglobin in erythrocytes in group B were significantly decreased (P <0.05), but there was no significant difference in the distribution width of erythrocytes (P> 0.05). The average volume of erythrocytes in group C was significantly increased, The difference was statistically significant (P <0.05), but there was no significant difference in the mean hemoglobin of erythrocytes and the width of erythrocyte distribution (P> 0.05). Conclusion In the diagnosis of anemia, blood test of erythrocyte parameters can be used to identify different types of anemia, which deserves further study and promotion.
