目的评价外周血细胞角蛋白19片段(CYFRA21-1)、神经特异性烯醇化酶(NSE)和癌胚抗原(CEA)联合检测对肺癌的临床诊断价值。方法采用化学免疫发光法测定105例肺癌患者、50例良性肺部疾病患者和40例健康人外周血CYFRA21-1、NSE和CEA水平;数据统计分析用t检验和χ2检验。结果肺癌组CYFRA 21-1、NSE和CEA阳性率及水平(分别为71.4%,39.0%,38%)明显高于良性肺部疾病组(分别为28%,16%,14%)和健康对照组(分别为10%,10%,12.5%)。随着临床分期逐步升高,CYFRA 21-1、NSE和CEA阳性率及水平升高。就组织类型而言,CYFRA21-1检测阳性率及表达水平以鳞癌最高;NSE阳性率及表达水平以小细胞癌最高;CEA阳性率及表达水平以腺癌最高。外周血CYFRA21-1、NSE和CEA联合检测可提高肺癌检出率。结论外周血CYFRA 21-1、NSE和CEA联合检测可提高肺癌检出率,并有助于判断其预后。
Objective To evaluate the clinical value of combined detection of cytokeratin 19 (CYFRA21-1), neuroendocrine enolase (NSE) and carcinoembryonic antigen (CEA) in the diagnosis of lung cancer. Methods The levels of CYFRA21-1, NSE and CEA in peripheral blood of 105 patients with lung cancer, 50 patients with benign lung disease and 40 healthy people were determined by chemiluminescence immunoassay. The data were analyzed by t-test and χ2 test. Results The positive rates and levels of CYFRA 21-1, NSE and CEA in lung cancer group were significantly higher than those in benign pulmonary disease group (71.4%, 39.0%, 38%, respectively) (28%, 16%, 14% Group (10%, 10%, 12.5%, respectively). With the gradual increase of clinical stage, CYFRA 21-1, NSE and CEA positive rate and level increased. In terms of tissue type, the positive rate and the expression level of CYFRA21-1 were highest in squamous cell carcinoma; the positive rate and expression level of NSE were the highest in small cell carcinoma; the positive rate and expression of CEA were the highest in adenocarcinoma. Peripheral blood CYFRA21-1, NSE and CEA combined detection can increase the detection rate of lung cancer. Conclusion Combined detection of CYFRA 21-1, NSE and CEA in peripheral blood can increase the detection rate of lung cancer and help to determine the prognosis.
