探讨HLA -DR .DQ基因多态性与骨关节结核的遗传关联性 ,比较骨关节结核与肺结核之间易感基因的差异。方法 采用聚合酶链反应 -序列特异性引物 (PCR -SSP)方法 ,对 86例骨关节结核 ,88例正常人及 34例肺结核的HLA -DR .DQ等位基因进行分析。结果 骨关节结核组与正常人比较 ,骨关节结核组DRB1 0 9、DQB1 0 30 1基因频率增高 (38.99%比 8.2 4 % PC <0 .0 0 0 1 RR =8.92 ;18.2 7%比 2 .6 6 % PC <0 .0 5 RR =2 .2 1) ,DRB1 13.2基因频率显著低于对照组 (4 .76 %比 2 0 .50 % PC <0 .0 0 1 RR =0 .18)。骨关节结核组与肺结核比较 ,DRB1 0 9、DQB1 0 30 1基因频率增高 (38.99%比 9.2 % PC <0 .0 0 0 1;18.15%比 1.85% PC <0 .0 0 1) ,而DRB1 15基因频率低于肺结核组 (17.17%比 36 .3% PC <0 .0 1)。结论 HLA -DRB1 0 9、DQB1 0 30 1可能是我国骨关节结核的易感基因 ,DRB1 13.2为保护基因。骨关节结核与肺结核之间易感基因存在差异
To investigate the genetic relationship between HLA-DR, DQ gene polymorphism and bone-and-joint tuberculosis and to compare the susceptibility genes between osteo-tuberculosis and tuberculosis. Methods The HLA-DR. DQ alleles of 86 cases of tuberculosis of joint, 88 cases of normal and 34 cases of pulmonary tuberculosis were analyzed by polymerase chain reaction-sequence-specific primers (PCR-SSP). Results The frequencies of DRB1 09 and DQB1 0301 gene in the bone-joint tuberculosis group were significantly higher than those in the normal group (38.99% vs 8.24%, PC0.001 RR = 8.92, 18.27% vs2. The frequency of DRB1 13.2 gene was significantly lower in the control group than in the control group (4.76% vs. 2.050% PC <0.0101 RR = 0.18) . Compared with tuberculosis, DRB1 0 9 and DQB1 0 30 1 gene frequencies were increased in osteoarticular tuberculosis group (38.99% vs 9.2% PC0.001; 18.15% vs 1.85% PC0.01), while DRB1 15 was lower than that of pulmonary tuberculosis (17.17% vs 36.3% PC <0.01). Conclusion HLA-DRB1 0 9 and DQB1 0 30 1 may be the susceptible genes of bone and joint tuberculosis in our country. DRB1 13.2 is the protective gene.
