目的评价替牙期骨性Ⅲ类错患儿行前方前牵引治疗后下颌骨的变化。方法选取山东大学口腔医院正畸科替牙期骨性Ⅲ类错患者60例,其中治疗组30例行前方前牵引治疗,对照组于观察期间未进行治疗。所有研究对象于治疗或观察前(T1)及治疗后或相应的观察期后(T2)拍摄X线头颅侧位片。在T1及T2时期所拍摄头颅侧位片上对反映上下颌骨水平及矢状向生长改变量的相关指标进行测量分析,并利用侧位片重叠技术对下颌骨位置的变化进行评价。结果经过6个月的前方前牵引治疗后,治疗组患者侧貌得到显著改善。治疗或观察期间,治疗组下颌骨发生顺时针旋转,平均旋转量为0.8°,对照组发生逆时针旋转,平均-0.6°,两者之间存在统计学差异(P<0.05);治疗组较对照组下颌骨水平生长量少。结论治疗组经前方前牵引治疗后下颌骨发生顺时针旋转,对照组下颌骨有逆时针生长趋势。
Objective To evaluate the changes of mandible in anterior skeletal class Ⅲ malocclusion patients before anterior approach. Methods Sixty cases of skeletal class Ⅲ malocclusion were selected from Department of Orthodontics, Stomatology Hospital of Shandong University. Among them, 30 patients in the treatment group underwent anterior traction and the control group were not treated during the observation period. All subjects underwent X-ray cephalograms before treatment or observation (T1) and after treatment or after the corresponding observation period (T2). During the T1 and T2 epilepsy, we measured the changes of the maxilla and mandible and the sagittal, and evaluated the changes of mandibular position by using the lateral overlap technique. Results After 6 months of anterior traction, the profile of the treated group was significantly improved. During the treatment or observation period, the mandibular bone of the treatment group rotated clockwise with an average rotation of 0.8 ° and the control group rotated counterclockwise with an average of -0.6 ° (P <0.05). There was significant difference between the two groups The control group mandibular horizontal growth less. Conclusion The mandibular incisors rotate clockwise when treated with anterior approach in the treatment group, while the mandibular bone in the control group has a counterclockwise growth trend.
