目的通过观察2型糖尿病(Type 2 diabetes,T2DM)患者脂联素(Adiponectin,APN)基因+45位点多态性及APN、炎性细胞因子水平,探讨APN基因+45位点与T2DM的关系。方法选择T2DM患者109例(T2DM组)及健康体检者110例(对照组)作为研究对象。检测2组对象生化指标,稳态模型评估法(HOMA)评估胰岛素抵抗指数(HOMA-IR),ELISA测定血清APN和炎性因子TNF-α和IL-1β水平,PCR-RFLP方法检测APN基因+45位点多态性。结果 T2DM患者APN基因+45位点T等位基因频率(62.84%)和TT基因型频率(44.95%)均明显低于对照组(P均<0.05);T2DM组患者炎性因子TNF-α和IL-1β以及生化指标FBG、FIN、TC、TG、LDL-C和HOMA-IR指标水平均高于对照组,而APN和HDL-C指标水平低于对照组,差异均有统计学意义(P<0.05);T2DM组患者TG和GG基因型组上述指标也明显不同于TT基因型组(P均<0.05);T2DM组患者血清APN水平与TNF-α、IL-1β、HOMA-IR呈明显负相关(P<0.05)。结论 T2DM患者存在APN基因+45位点多态性,T→G突变能影响血清APN水平、炎性细胞因子和胰岛素抵抗,共同参与T2DM的发生。
Objective To investigate the relationship between APN +45 locus and T2DM by observing the polymorphism of +45 locus of Adiponectin (APN) and the levels of APN and inflammatory cytokines in patients with type 2 diabetes (T2DM) . Methods Totally 109 patients with T2DM (T2DM group) and 110 healthy controls (control group) were selected as the study subjects. HOMA-IR, serum APN and TNF-α and IL-1β levels in serum were detected by ELISA. The levels of APN gene + 45-site polymorphism. Results The frequencies of T allele at +45 locus (62.84%) and TT genotype (44.95%) in T2DM patients were significantly lower than those in control group (all P <0.05). The levels of TNF-α and TNF- IL-1β and biochemical indicators FBG, FIN, TC, TG, LDL-C and HOMA-IR index levels were higher than the control group, while APN and HDL-C index levels were lower than the control group, the difference was statistically significant (P <0.05). The levels of TG and GG in T2DM group were also significantly different from those in TT genotype group (P <0.05). The levels of serum APN and TNF-α, IL-1β and HOMA-IR in T2DM group were significantly higher than those in TT genotype group Negative correlation (P <0.05). Conclusion The APN +45 locus polymorphism exists in T2DM patients. T → G mutation can affect serum APN levels, inflammatory cytokines and insulin resistance, and participate in the occurrence of T2DM.
