目的探讨瞬时受体电位离子通道(TRPM7)在胆管癌组织中的表达及其与预后的相关性。方法应用免疫组织化学SP法检测TRPM7在49例胆管癌、7例胆管良性病变组织和36例癌旁正常胆管组织的表达情况,分析其与胆管癌临床病理的关系。结果胆管癌组织中TRPM7表达阳性率为77.6%(38/49),高于其在胆管良性病变组织的表达阳性率(0,0/7)及癌旁正常胆管组织中的表达阳性率(2.8%,1/36),差异均有统计学意义(P<0.05)。TRPM7蛋白的表达与胆管癌的TNM分期、淋巴结转移和脏器转移相关(P<0.05),但与患者的年龄、性别、部位、肿瘤分化程度及肝炎无相关性(P>0.05)。Kaplan-Meier生存分析发现TRPM7阳性表达患者的生存期低于TRPM7阴性表达患者(P<0.05)。Cox多因素分析显示,TRPM7的表达水平与生存预后显著相关(P<0.05),是影响预后的独立危险因素。结论 TRPM7在胆管癌的发生、发展和浸润转移中发挥着重要作用,是影响胆管癌患者预后的重要因素。
Objective To investigate the expression of transient receptor potential ion channel (TRPM7) in cholangiocarcinoma and its correlation with prognosis. Methods The expression of TRPM7 in 49 cases of cholangiocarcinoma, 7 cases of benign biliary lesions and 36 cases of normal bile duct tissues was detected by immunohistochemical SP method. The relationship between TRPM7 expression and the clinicopathological features of cholangiocarcinoma was analyzed. Results The positive rate of TRPM7 expression in cholangiocarcinoma was 77.6% (38/49), which was higher than that in the benign biliary lesions (0, 0/7) and normal paracancerous tissues %, 1/36), the differences were statistically significant (P <0.05). The expression of TRPM7 protein was correlated with TNM stage, lymph node metastasis and organ metastasis of cholangiocarcinoma (P <0.05), but not with the age, sex, location, tumor differentiation and hepatitis (P> 0.05). Kaplan-Meier survival analysis showed that the survival of patients with TRPM7 positive expression was lower than that of patients with TRPM7 negative expression (P <0.05). Cox multivariate analysis showed that the expression level of TRPM7 was significantly correlated with survival prognosis (P <0.05), which was an independent risk factor for prognosis. Conclusion TRPM7 plays an important role in the occurrence, development and invasion and metastasis of cholangiocarcinoma and is an important factor affecting the prognosis of patients with cholangiocarcinoma.
