目的应用超声心动图评价急性ST段抬高型心肌梗死(STEMI)患者急诊经皮冠状动脉内介入术(PCI)血管开通前后E/E′与左室舒张末压(LVEDP)的相关性,探讨E/E′在对STEMI患者急诊PCI术前、术后即刻左室舒张功能评价中的价值。方法对入选的39例前壁STEMI患者分别于急诊PCI术前、术后即刻行超声心动图检查。检测指标:左室舒张末期前后径(LVDD)、左房容积(LAV)、左心室射血分数(LVEF)、二尖瓣口舒张期血流E峰、A峰,二尖瓣E峰减速时间(DT);二尖瓣环后间隔、侧壁、下壁三个位点的舒张早期运动速度E′、计算E/E′。结果 (1)STEMI患者急诊PCI术前E/E′与左室舒张末压无显著相关性;(2)STEMI患者急诊PCI术后即刻E/E′与左室舒张末压无显著相关性;(3)急诊PCI术前、术后E/E′分别与术前、术后的LVEF、LVDD、LAV、E/A、DT的相关性分析中,术前的E/E′与LAV有显著相关性;(4)急诊PCI术前、术后平均E/E′及LVEDP比较中,术前、术后的LVEDP比较有显著差异。结论 E/E′与STEMI患者急诊PCI血管开通前后的LVEDP无显著相关性,E/E′不能用于对STEMI患者急诊PCI术前、术后即刻左室舒张功能的评价。E/E′可能与长期的左室舒张功能状态相关。
Objective To evaluate the relationship between E / E ’and left ventricular end diastolic pressure (LVEDP) before and after percutaneous coronary intervention (PCI) in patients with acute ST-segment elevation myocardial infarction (STEMI) by echocardiography E / E ’in assessing left ventricular diastolic function before and immediately after PCI for STEMI patients. Methods Thirty-nine patients with anterior wall STEMI were examined by echocardiography immediately before and immediately after PCI. Measurements of left ventricular end-diastolic anteroposterior diameter (LVDD), left atrial volume (LAV), left ventricular ejection fraction (LVEF), mitral valve diastolic flow E peak, peak A, mitral E peak deceleration time (DT); mitral annulus after the interval, the wall, the lower wall of the three early diastolic velocity E ’, calculate E / E’. Results (1) There was no significant correlation between E / E ’and left ventricular end-diastolic pressure before PCI in STEMI patients. (2) There was no significant correlation between E / E’ and left ventricular end-diastolic pressure immediately after PCI in STEMI patients. (3) Preoperative E / E ’and postoperative E / E’ were correlated with preoperative and postoperative LVEF, LVDD, LAV, E / A and DT (4) The preoperative and postoperative LVEDP were significantly different between the preoperative and postoperative mean E / E ’and LVEDP. Conclusions There is no significant correlation between E / E ’and LVEDP in patients with STEMI before and after PCI. E / E’ can not be used to evaluate left ventricular diastolic function in STEMI patients before and after PCI. E / E ’may be related to long-term state of left ventricular diastolic function.
