应用99MTc-MIBI对23例系统性硬皮病(SSC)病人进行了心肌断层显像冷诱导试验,并同期应用二维超声心动图检测左心室功能和室壁运动。结果表明,14例病人(60.9%)有心肌放射性分布异常,其中3例可逆性缺损6个节段,5例固定性缺损9个节段,6例两种缺损共存14个节段;心肌显像异常率在雷诺现象(RP)组和无RP组之间无显著性差异(0.5>P>0.25);心肌显像异常组病人的左室射血分数、每搏输出量、心输出量在冷试验即刻较静息时显著降低(P<0.05),且与心肌显像正常组比较也有显著性降低(P<0.05);超声心动图示4例病人有局部室壁运动低下,部位与心肌图像上可逆性缺损区域一致。提示应用心肌断层显像冷试验估价SSC病人的心肌损害有较高的临床价值。
Twenty-three patients with systemic scleroderma (SSC) were subjected to myocardial ischemia imaging with 99MTc-MIBI. Left ventricular function and ventricular wall motion were measured by two-dimensional echocardiography during the same period. The results showed that in 14 patients (60.9%), the myocardial radioactivity was abnormal. Among them, 3 cases had reversible defect 6 segments, 5 fixed defects 9 segments and 6 cases both had 14 segments. There was no significant difference in myocardial imaging abnormalities between Renault phenomenon (RP) group and non-RP group (0.5> P> 0.25). Left ventricular ejection fraction, stroke output Volume and cardiac output were significantly lower (P <0.05) immediately after the cold test than at rest, and also significantly decreased compared with the normal group (P <0.05). Echocardiography showed that 4 patients Local wall motion is low, the site consistent with the reversible defect in myocardial images. It is suggested that the application of myocardial perfusion imaging cold test in patients with SSC myocardial damage has a higher clinical value.
