我科自1982年以来,开展保留牙髓行牙再植和自体牙移植20例,并行临床追踪观察1～2.5年以上,效果满意。现介绍如下:一、临床资料:在保留牙髓的20例植牙中,自体牙移植1例,再植19例。其中:中切牙8例,侧切牙7例,单尖牙5例。属外伤离体牙再植17例(包括离体一小时以上被泥土污染的牙),正畸2例,牙周病1例。植牙者年龄在11岁至40岁之间。追踪观察1～2.5年,18例再植牙和1例移植牙生长良好,牙色正常,冷热感觉存在,咀嚼功能正常。1例牙周病再植后终因炎症而失败。二、操作方法:先作普鲁卡因和青霉素皮试,在局麻下手术。拔下供牙后浸泡于青霉素生理盐水中,被污染了的离体牙先行清洗后再浸泡于上液中。植牙时从浸泡液中取供牙植入,以银丝行“8”形交叉结扎加夹板固定,并调整降低咬(牙合)关系。移植牙植入前先修整牙槽窝。
Since 1982, our department has carried out 20 cases of dental pulp replantation and autologous dental grafts and observed with clinical follow-up for 1 to 2.5 years with satisfactory results. Now introduced as follows: First, the clinical data: in retaining the pulp of 20 cases of dental implants, autologous dental transplantation in 1 case, replantation in 19 cases. Including: 8 cases of incisors, lateral incisors in 7 cases, 5 cases of monochoria. A case of traumatic isolated dental replantation in 17 cases (including teeth contaminated by soil for more than an hour in vitro), 2 cases of orthodontics, 1 case of periodontal disease. Implant users are between the ages of 11 and 40. Tracing the observation for 1-2.5 years, 18 cases of replantation and 1 case of dental implants grew well, the tooth color was normal, the feeling of hot and cold was present, and the chewing function was normal. One case of periodontal disease eventually failed due to inflammation after replantation. Second, methods of operation: first for procaine and penicillin skin test, under local anesthesia surgery. Unplug the tooth soaked in penicillin saline, the contaminated tooth isolated before cleaning and soaking in the liquid. Implantation from the infusion solution for dental implants to silver wire “8” cross ligation plus plywood fixed, and adjust to reduce the occlusion relationship. Dress the alveolar socket before implanting.
