目的 :探讨原发性高血压 (EH)患者尿 β2 微球蛋白 (β2 MG)的主要影响因素及其意义。方法 :选择不伴有糖尿病、原发性肾脏损害的EH患者 91例 ,采用放射免疫法测定早晨第 1次尿中 β2 MG ,将其分为 β2 MG正常 (对照 )和异常两组 ,并测定所有患者血压、血脂、空腹血糖及其他一般情况 ,进行统计分析。运用多元线性回归识别影响 β2 MG的主要因素。结果 :尿 β2 MG异常组 ,血清总胆固醇、三酰甘油、低密度脂蛋白胆固醇、脉压(PP)高于尿 β2 MG正常组 (P <0 .0 5 ) ,高密度脂蛋白胆固醇低于尿 β2 MG正常组 (P <0 .0 5 )。回归分析结果上述血脂各项指标及PP均未进入回归方程 ,只有年龄、病程及收缩压 (SBP)进入方程。结论 :在不伴有糖尿病及原发性肾脏疾病的EH患者中 ,脂质代谢紊乱及PP在早期肾脏损害中起一定作用 ,但不是主要的。影响早期肾脏损害的主要因素是年龄、病程及SBP。
Objective: To investigate the main influencing factors of urinary β2 microglobulin (β2 MG) in patients with essential hypertension (EH) and its significance. Methods: Ninety patients with EH without diabetes and primary renal damage were selected. Β2 MG in the first morning urine was determined by radioimmunoassay and divided into two groups: normal (control) and abnormal All patients with blood pressure, blood lipids, fasting blood glucose and other general conditions, for statistical analysis. Multiple linear regression was used to identify the main factors influencing β2 MG. Results: Serum total cholesterol, triglyceride, low density lipoprotein cholesterol, and pulse pressure (PP) were higher in urine β2 MG abnormal group than those in normal urine β2 MG group (P <0.05), and high density lipoprotein cholesterol Urinary β2 MG normal group (P <0.05). Regression analysis of the above indicators of blood lipids and PP did not enter the regression equation, only age, duration and systolic blood pressure (SBP) into the equation. Conclusions: In EH patients without diabetes and primary kidney disease, dyslipidemia and PP play a role, but not major, in early kidney damage. The main factors affecting early renal damage are age, duration of disease and SBP.
