目的 探讨原发性高血压患者血清胰岛素样生长因子 (IGF )、生长激素 (GH )和胰岛素(IS)变化及其临床意义。方法 用放射免疫法测定 92例原发性高血压 (EH)患者和 4 5例非高血压患者的血清 IGF 、 GH和 IS水平 ,并进行对照统计分析。结果 EH组血清 IGF 和 IS水平显著高于对照组 (P<0 .0 1) ,而 GH水平显著低于对照组 ,IGF 和 GH、GH和 IS间呈显著负相关 (P<0 .0 5 ) ,IGF 和 IS与平均动脉压成显著正相关 (P<0 .0 5 ) ,GH与平均动脉压无显著相关性。男女组间均值比较均无统计学差异。在 、 、 期组间 ,血清 GH水平依次递减 (方差分析 ,P<0 .0 1) ,且 期组显著低于 期组 (P<0 .0 5 ) ,伴心脑肾并发症组血清 GH水平亦显著低于无并发症组 (P<0 .0 5 ) ;血清 IGF 和 IS水平则均无统计学意义。结论 EH患者血清 IGF 和 IS水平显著升高 ,而 GH水平显著降低 ,GH和 IGF 、GH和 IS负相关 ,且 IGF 和 IS与平均动脉压正相关 ,三者变化与性别无关 ,随着病情进展 ,GH有进一步降低趋势 ,但 IGF 和 IS无此趋势。
Objective To investigate the changes of serum insulin-like growth factor (IGF), growth hormone (GH) and insulin (IS) in patients with essential hypertension and its clinical significance. Methods Serum levels of IGF, GH and IS were measured in 92 patients with essential hypertension (EH) and 45 non-hypertensive patients by radioimmunoassay. Statistical analysis was performed. Results Serum levels of IGF and IS in EH group were significantly higher than those in control group (P <0.01), while GH levels in EH group were significantly lower than those in control group (P <0.05) ). There was a significant positive correlation between IGF and IS and mean arterial pressure (P <0.05), and no significant correlation between GH and mean arterial pressure. There was no significant difference in mean between men and women. Serum levels of GH decreased gradually (P <0.01), and the level of serum GH was significantly lower in the period group than in the period group (P <0.05) The levels were also significantly lower than those without complications (P <0.05), while the levels of serum IGF and IS were not statistically significant. Conclusions Serum levels of IGF and IS were significantly increased in EH patients, GH levels were significantly lower, GH was negatively correlated with IGF, GH and IS, and IGF and IS were positively correlated with mean arterial pressure. The changes of the three were not related to sex, , GH further reduce the trend, but IGF and IS no such trend.
