目的就纳洛酮对呼吸衰竭肺性脑病患者的临床疗效进行探讨。方法选取我院自2009年1月至2012年1月之间所收治的80例呼吸衰竭肺性脑病患者作为调查对象,将其随机分为治疗组(40例)和对照组(40例)。40例对照组患者给予一系列的常规治疗和对症处理(维持水电解质平衡、保证营养、化痰、止咳、平喘、吸氧、呼吸道通畅等)。而40例治疗组患者在此基础上加用纳洛酮持续静脉点滴,纳洛酮由北京四环医药科技股份公司生产,3天为一疗程。结果例治疗组患者显效27例,有效10例,无效3例,总有效率为92.5%;而40例对照组患者显效10例,有效19例,无效11例,总有效率为72.5%,二者具有较为明显的统计学差异,P=0.003<0.05。两组都没有在治疗的过程中出现肾功能损害、肝功能损害、过敏、皮疹、血压波动等不良反应。结论纳洛酮可以有效改善肺性脑病患者的昏迷、抽搐、精神错乱、呼吸困难、电解质紊乱、发绀等症状,不良反应也少,能够大幅度降低患者家庭的经济负担,减少住院费用和脑组织损害,有利于生活质量提高和后期的身体恢复,值得在临床中大量应用。
Objective To investigate the clinical efficacy of naloxone in patients with respiratory failure pulmonary encephalopathy. Methods Eighty patients with respiratory failure pulmonary encephalopathy admitted in our hospital from January 2009 to January 2012 were randomly divided into treatment group (40 cases) and control group (40 cases). Forty patients in the control group were given a series of routine treatment and symptomatic treatment (to maintain the balance of water and electrolyte, to ensure nutrition, phlegm, cough, asthma, oxygen, airway patency, etc.). On the basis of this, 40 patients in the treatment group continued to receive intravenous naloxone intravenously. Naloxone was produced by Beijing Sihuan Pharmaceutical Science & Technology Co., Ltd. for 3 days. Results In the treatment group, 27 cases were markedly effective, 10 cases were effective, 3 cases were ineffective, and the total effective rate was 92.5%. In 40 cases of control group, 10 cases were markedly effective, 19 cases were effective and 11 cases were ineffective. The total effective rate was 72.5% Who had a more significant statistical difference, P = 0.003 <0.05. Neither group had any adverse effects of renal dysfunction, liver function impairment, allergy, rash, blood pressure fluctuations during treatment. Conclusion Naloxone can effectively improve the coma, convulsion, delirium, dyspnea, electrolyte disturbance, cyanosis and other symptoms in patients with pulmonary encephalopathy, with less adverse reactions. It can significantly reduce the financial burden on families and reduce the cost of hospitalization and brain tissue Damage, is conducive to improving the quality of life and the recovery of the body later, it is worth a large number of clinical applications.
