目的探讨穿孔素基因对HLH患儿血清IL-18、IL-10、IL-12、TNF-α、NF-Kβ及新碟呤的影响。方法应用酶联免疫吸附法(ELISA)检测20例HLH患儿确诊时、化疗第1~2周、第2~3周、第3~4周、第5~7周及8~10周,共6组的血清IL-18、IL-10、IL-12、TNF-α、NF-Kβ及新碟呤水平变化,11例患儿穿孔素基因阴性,5例患儿穿孔素基因阳性,对穿孔素阴性组和阳性组的细胞因子水平进行比较。结果穿孔素阳性组的IL-10、IL-12、TNF-α水平明显高于穿孔素阴性组,两组相比差异有统计学意义(P<0.01)。穿孔素阳性组的IL-18水平高于穿孔素阴性组,两组相比差异有统计学意义(P<0.05)。穿孔素阳性组与穿孔素阴性组相比新碟呤水平无差异,两组相比差异无统计学意义(P>0.05)。结论穿孔素基因影响HLH患儿IL-10、IL-12、TNF-α及IL-18的表达。
Objective To investigate the effects of perforin on serum IL-18, IL-10, IL-12, TNF-α, NF-κβ and neopterin in children with HLH. Methods Twenty cases of HLH were diagnosed by enzyme-linked immunosorbent assay (ELISA) at weeks 1 to 2, 2 to 3, 3 to 4, 5 to 7, and 8 to 10 weeks after chemotherapy The serum levels of IL-18, IL-10, IL-12, TNF-α, NF-κB and neopterin in 6 groups were changed, 11 cases were negative for perforin, 5 cases were positive for perforin, The levels of cytokines in the negative and positive groups were compared. Results The levels of IL-10, IL-12 and TNF-α in perforin-positive group were significantly higher than those in perforin-negative group. There was significant difference between the two groups (P <0.01). Perforin-positive group of IL-18 levels higher than perforin-negative group, the difference between the two groups was statistically significant (P <0.05). There was no difference in neopterin between perforin-positive group and perforin-negative group. There was no significant difference between the two groups (P> 0.05). Conclusion Perforin gene affects the expression of IL-10, IL-12, TNF-α and IL-18 in children with HLH.
