探讨Ⅰ型糖尿病患者植物神经病变和血压与糖尿病肾病的关系。方法:对44例偶测血压正常的Ⅰ型糖尿病患者进行了心脏植物神经功能检查、24小时动态血压监测(ABPM)以及尿白蛋白排泄率(UAE)测定。结果:存在植物神经病变的Ⅰ型糖尿病患者其偶测收缩压、舒张压、UAE均高于无植物神经病变者,且ABPM多项指标异常。心脏植物神经功能检查计分与UAE及多项ABPM指标显著相关,但与偶测血压无相关性。结论:偶测血压正常的Ⅰ型糖尿病患者植物神经病变、24小时血压与UAE之间具有一定的关联。
To investigate the relationship between autonomic neuropathy and blood pressure and diabetic nephropathy in type 1 diabetic patients. Methods: Forty-four patients with type I diabetes mellitus who had normal blood pressure were examined for cardiac autonomic function, 24-hour ambulatory blood pressure monitoring (ABPM) and urinary albumin excretion rate (UAE). Results: In patients with type 1 diabetes mellitus with autonomic neuropathy, the measured systolic blood pressure, diastolic blood pressure, UAE were higher than those without autonomic neuropathy, and many indicators of ABPM were abnormal. Cardiac autonomic nerve function test scores were significantly associated with UAE and a number of ABPM parameters, but no correlation with even measured blood pressure. CONCLUSION: Autonomic neuropathy in patients with type I diabetes mellitus with even normotensive blood pressure correlates with UAE at 24 hours.
