目的:评价青少年安氏Ⅱ类2分类错牙合患者正畸治疗前后颅颌面结构的变化。方法:测量分析12例青少年安氏Ⅱ类2分类错牙合患者正畸治疗前后的头颅定位侧位X线片上的各项指标。结果:正畸治疗后,前颅底长增加,上颌骨矢状向长度无明显变化,下颌骨体部、升支及下颌骨总长度增加,上、下、前、后面高增大;腭平面、下颌平面无明显旋转,前后面高比值无显著变化;SNA减小,SNB无显著变化,ANB角减小,Y轴角增大。SNB在治疗前后的差值与下颌骨总长、体部、升支治疗前后差值呈显著正相关,ANB在治疗前后的差值与下颌骨总长及升支治疗前后差值呈显著负相关。结论:正畸治疗可以改善青少年患者颅颌面结构缺陷。下颌骨垂直向以及水平向的生长有助于上下颌骨Ⅱ类关系的改善。
Objective: To evaluate the changes of craniomaxillofacial structure before and after orthodontic treatment in adolescents with Class Ⅱ division 2 malocclusion. Methods: The indexes of skull position X - ray before and after orthodontic treatment in 12 adolescent Class Ⅱ division malocclusion patients were measured and analyzed. Results: After orthodontic treatment, the length of anterior cranial base increased and the sagittal length of maxillary did not change significantly. The total length of mandibular body, ascending branch and mandible increased, and the height of upper, lower, anterior and posterior surfaces increased. , No significant rotation of the mandibular plane, anteroposterior height ratio no significant change; SNA decreases, SNB no significant change, ANB angle decreases, Y axis angle increases. The difference of SNB before and after treatment was positively correlated with the total length of mandible, body and ascending branch before and after treatment, and the difference of ANB before and after treatment was negatively correlated with the total length of mandible and the difference between before and after treatment. Conclusion: Orthodontic treatment can improve craniofacial structural defects in adolescent patients. Mandibular vertical and horizontal growth contribute to the improvement of the relationship between the upper and lower jaw Class Ⅱ.
