目的探讨改良注射用亚胺培南西司他丁钠输注方案治疗ICU获得性鲍氏不动杆菌肺炎的临床疗效。方法选取2013年5月—2014年5月湘雅萍矿合作医院收治的ICU获得性鲍氏不动杆菌肺炎患者96例,按治疗方案不同将其分为试验组(50例)与对照组(46例)。试验组患者予以改良注射用亚胺培南西司他丁钠输注方案治疗,对照组患者予以常规注射用亚胺培南西司他丁钠输注方案治疗。观察两组患者治疗3、5、7d成功率、治疗后复发率、治疗时间及治疗费用。结果两组患者治疗3、5、7d成功率比较,差异无统计学意义(P>0.05);两组患者治疗后复发率比较,差异无统计学意义(P>0.05);两组患者治疗后鲍氏不动杆菌对注射用亚胺培南西司他丁钠MIC90结果为2μg/ml,未发生变化,细菌培养均未发现泛耐药性革兰阴性杆菌;两组患者治疗时间比较,差异无统计学意义(P>0.05),试验组患者治疗费用低于对照组,差异有统计学意义(P<0.05)。结论改良注射用亚胺培南西司他丁钠输注方案治疗ICU获得性鲍氏不动杆菌肺炎与传统注射用亚胺培南西司他丁钠输注方案的临床疗效相当,而改良注射用亚胺培南西司他丁钠输注方案可降低患者治疗费用。
Objective To investigate the clinical efficacy of modified infusion imipenem cilastatin sodium infusion in the treatment of ICU Acquired Acinetobacter baumannii pneumonia. Methods Ninety-six patients with ICU Acquired Acinetobacter baumannii pneumonia admitted from May 2013 to May 2014 in Xiangyaping Mine Cooperative Hospital were divided into experimental group (50 cases) and control group 46 cases). Patients in the test group were treated with imipenem and cilastatin sodium infusion. The patients in the control group were treated with imipenem and cilastatin sodium infusion routinely. The success rate of 3,5,7d treatment, relapse rate, treatment time and treatment cost were observed in the two groups. Results There was no significant difference in the success rates between the two groups at 3,5 and 7 days (P> 0.05). There was no significant difference in the recurrence rate between the two groups after treatment (P> 0.05) Acinetobacter baumannii for injection of imipenem cilastatin sodium MIC90 results 2μg / ml, did not change, no pan-drug-resistant Gram-negative bacilli were found in bacterial culture; two groups of patients with treatment time, the difference was no Statistical significance (P> 0.05), the experimental group patients treatment costs lower than the control group, the difference was statistically significant (P <0.05). Conclusion The improved IM injection of imipenem cilastatin sodium infusion program for the treatment of ICU Acquired Acinetobacter baumannii pneumonia and traditional injection of imipenem cilastatin sodium infusion of clinical efficacy of the program, Amopenec West Statinar sodium infusion program can reduce the cost of treatment of patients.
