目的探究幽门螺杆菌(Hp)感染与胃癌微转移的相关性。方法选取2011年10月至2012年12月间甘肃省人民医院收治的64例胃癌患者和10例胃溃疡等良性病变患者,检测所有患者Hp,依结果分为Hp(+)组与Hp(-)组。采集患者外周血,使用流式细胞仪测定细胞因子CK19和CK20表达水平,并比较胃癌和胃溃疡等良性病变患者Hp(+)组与Hp(-)组外周血中CK19和CK20的表达水平。结果良性病变患者外周血中均未检出CK19和CK20。Hp(+)胃癌患者外周血中CK19阳性表达率为54.3%,CK20阳性表达率为51.4%,Hp(-)胃癌患者外周血中CK19阳性表达率为27.6%,CK20阳性表达率为34.5%,Hp(+)胃癌患者CK19和CK20表达均高于Hp(-)胃癌患者,但前者差异有统计学意义(P<0.05),后者差异无统计学意义(P>0.05)。结论 Hp感染使胃癌患者外周血中细胞因子高表达,可能促进胃癌微转移的发生。
Objective To investigate the relationship between Helicobacter pylori (Hp) infection and gastric micrometastasis. Methods Totally 64 patients with gastric cancer and 10 patients with benign lesions such as gastric ulcer were enrolled in the Gansu Provincial People ’s Hospital from October 2011 to December 2012. Hp was detected in all patients with Hp (+) and Hp (- )group. Peripheral blood samples were collected and the expression of CK19 and CK20 were measured by flow cytometry. The expressions of CK19 and CK20 in Hp (+) and Hp (-) groups were compared between gastric cancer and gastric ulcer patients. Results CK19 and CK20 were not detected in the peripheral blood of patients with benign lesions. The positive rate of CK19 in peripheral blood of Hp (+) gastric cancer patients was 54.3%, the positive rate of CK20 was 51.4%, the positive rate of CK19 in peripheral blood of Hp (-) gastric cancer patients was 27.6%, the positive rate of CK20 was 34.5% The expression of CK19 and CK20 in patients with Hp (+) gastric cancer was higher than that in patients with Hp (-) gastric cancer, but the difference was statistically significant (P <0.05). The difference was not statistically significant (P> 0.05). Conclusions Hp infection can increase the expression of cytokines in peripheral blood of patients with gastric cancer, which may promote the occurrence of gastric cancer micrometastasis.
