目的:分析超低出生体质量儿配方乳喂养中胃潴留与全肠道喂养的关系,探讨超低出生体质量儿配方乳喂养的全过程。方法:选取2013年6月至2015年6月在本院出生的超低出生体质量儿22例,所有患儿采用早产儿配方乳喂养,先非营养性喂养,酌情逐量增加奶量,直至全胃肠道喂养。记录出现胃内潴留、胃内潴留(喂养不耐受)时间、全胃肠道平均时间、自动吸吮时间等,并采用多元线性回归分析讨论两个变量的影响关系。结果:本组22例患儿在按照早产儿配方奶进行喂养过程中,有1例出现坏死性小肠结肠炎;1例出现严重喂养不耐受,持续时间为26 d;21例完成全胃肠道喂养,首日开奶成功的患儿12例,开奶成功率为57.1%;达到胃肠道营养的平均时间为30.1 d;喂养胃内潴留量和达全胃肠道营养时间无相关性(P>0.05),喂养不耐受潴留量和自主吸吮时间无相关性(P>0.05)。结论:超低出生体质量儿早产儿配方乳喂养的过程持续时间较长,并且在喂养早期喂养不耐受的发生率较高,在喂养不耐受患儿1周后患儿奶量才能逐渐增加,可从喂养第2周实行更积极喂养,可根据喂养不耐受时间推断患儿达全胃肠道喂养时间和自动吸吮时间。
OBJECTIVE: To analyze the relationship between gastric retention and total gut feeding in formula feeding of ultra-low birth weight children and to explore the whole process of formula feeding with ultra-low birth weight children. Methods: Twenty-two cases of ultra-low birth weight (BMI) born in our hospital from June 2013 to June 2015 were selected.
