目的分析血清C肽联合糖化血红蛋白检测对糖尿病诊断的临床价值。方法收集医院内分泌科收治的糖尿病患者80例,根据病情将单纯糖尿病患者55例为糖尿病组,合并肾病的糖尿病患者25例为糖尿病肾病组,同时筛选同期医院体检中心健康者30例作为健康对照组,检测各组糖化血红蛋白(Hb A1c)、空腹血糖(FBG)、餐后2h血糖、血清C肽等指标,分析Hb A1c与FBG、C肽相关性。结果糖尿病组与糖尿病肾病组患者的血清Hb A1c、FBG、餐后2 h血糖水平均高于健康对照组,血清C肽水平低于健康对照组,差异均有统计学意义(P<0.05或P<0.01);糖尿病组与糖尿病肾病组FBG、餐后2 h血糖水平相比,差异无统计学意义(P>0.05);糖尿病肾病组Hb A1c水平高于糖尿病组,且C肽水平低于糖尿病组(P<0.05)。Hb A1c与FBG呈正相关(r=0.444,P<0.05),与血清C肽呈负相关(r=-0.627,P<0.05)。结论血清C肽联合Hb A1c可有效反映患者血糖控制水平及病情严重程度,对糖尿病的早期诊断、治疗及预后的监测具有重要作用,值得临床应用。
Objective To analyze the clinical value of serum C-peptide combined with glycosylated hemoglobin in the diagnosis of diabetes. Methods 80 cases of diabetic patients admitted to the hospital endocrine department were collected. According to the condition, 55 cases of simple diabetic patients were diabetic group, 25 cases of diabetic patients with nephropathy were diabetic nephropathy group, and 30 healthy subjects were selected as healthy control group (Hb A1c), fasting blood glucose (FBG), postprandial blood glucose 2h, serum C-peptide and other indexes were detected to determine the correlation between Hb A1c and FBG and C peptide. Results The serum levels of Hb A1c, FBG and postprandial 2h blood glucose were significantly higher in patients with diabetes mellitus and diabetic nephropathy than those in healthy controls, and the serum C-peptide levels were lower than those in healthy controls (P <0.05 or P <0.01). There was no significant difference in the FBG level between the diabetic group and the diabetic nephropathy group at 2 h after meal (P> 0.05). The level of Hb A1c in diabetic nephropathy group was higher than that in the diabetic group, and the level of C peptide was lower than that in the diabetic group Group (P <0.05). Hb A1c was positively correlated with FBG (r = 0.444, P <0.05), but negatively correlated with serum C peptide (r = -0.627, P <0.05). Conclusion Serum C-peptide combined with Hb A1c can effectively reflect the level of blood glucose control and the severity of the disease. It plays an important role in the early diagnosis, treatment and prognosis of diabetes and is worthy of clinical application.
