先天性小腿环状缩窄合并足畸形的治疗

目的 :对我院近 10年来治疗先天性小腿环状缩窄合并足畸形的手术方法和疗效进行总结。方法 :小腿环状缩窄的矫治分二阶段进行 ,第一阶段的手术时间为生后 2~ 7个月 ,施行一期环状缩窄切除及多个“Z”形转移皮瓣成形术 ,第二阶段的手术主要是矫治足的畸形 ,手术时间为 2~ 6岁 ,施行跟腱延长、Turco手术和石膏矫形。结果 :本组患儿均获得随访 ,马蹄内翻足、足内翻和扁平足各 1例随访时已痊愈 ,足循环障碍在第一阶段手术后已恢复循环。除先天性截肢外其余患儿均不影响足功能和足外形。结论 :小腿环状缩窄合并足畸形应争取早期施行环状缩窄切除术 ,以解除压迫在缩窄环下的主要外周神经 ,有利于足畸形的早期自愈。如果第一阶段术后足畸形好转不明显 ,应及早行第二阶段手术 ,以免患儿站立行走后足畸形加重。 Objective: To summarize the surgical methods and curative effects of congenital lower limb ring deformity in the past 10 years in our hospital. Methods: The correction of annular reduction of the calf was performed in two stages. The first stage of operation time was 2 to 7 months after birth. One cycle of annular narrowing and multiple “Z” transfer flap angioplasty were performed. The second phase of surgery is mainly the correction of foot deformity, operation time is 2 to 6 years old, the implementation of Achilles tendon extension, Turco surgery and plaster correction. Results: All the patients were followed up. One case of clubfoot, foot varus and flat foot had been recovered at follow - up. Foot circulation disorder was recovered after the first stage of operation. Except congenital amputation, the rest of the children did not affect foot function and foot shape. CONCLUSIONS: The ring-shaped narrowing of the foot combined with foot deformity should strive for the early implementation of ring-shaped narrowing resection to relieve the compression of the major peripheral nerves under the narrowing ring, which is good for the early self-healing of foot deformities. If the first stage after the improvement of foot deformity is not obvious, should be the second phase of surgery as soon as possible, so as to avoid foot walking in children with foot deformity worsened.
